Clearing a worker to return to a desk job and clearing them to return to a construction site are not the same decision. And yet IME referrals for physical roles are often structured exactly the same way - generic questions, single specialty, a medical clearance that isn't really a return-to-work plan. For trades, healthcare, warehouse, and construction roles, the margin for error is much narrower. The wrong call means re-injury, operational disruption, and a file that's back on your desk within months. Some files don't fit neatly inside one specialty. A person with a chronic pain presentation, a depression diagnosis, an unresolved orthopaedic concern, and ongoing neurological symptoms is not a one-assessor file. It's a coordination problem before it's a clinical one. Multi-specialty files require sequenced bookings, shared documentation, and reports that talk to each other rather than past each other. PTSD is real. It's also one of the most clinically complex diagnoses to assess well. The question that matters most in a PTSD file isn't just "does this claimant have PTSD?" It's whether your report actually distinguishes PTSD from adjustment disorder, complex trauma, depression with trauma features - and can hold up when it's challenged. Most don't go there.
