Few publications on primary Trigeminal Neuralgia treated by Micro-Vascular Decompression (MVD) report large series, with long-term follow-up, using Kaplan-Meier (K-M) analysis. None was specifically directed to the comparative study of MVD effectiveness on Trigeminal Neuralgia with typical (i.e., with paroxysmal pain only) and atypical features (i.e., with association of a permanent background of pain). Method. The authors report a series of 362 patients having clearcut vascular compression and treated with pure MVD-i.e., without any additional cut or coagulation of the adjacent root fibers. Follow-up was 1 to 18 y (8 y on average, with a median of 7.2 y). Results were considered overall, then separately for patients with typical (237 (65.5%)) and atypical (125 (34.5%)) clinical presentation. Findings. One year after operation, (294 (81.2%) of patients were totally-free-of paroxysmal pain, and also of permanent background pain-and not needing any medication) 13 (3.6%) still had a background of pain but without the need for medication which 55 patients (15.2%), treatment had failed. At latest review (8 y on average) the corresponding rates were 80, 4.9 and 15.1%, respectively. Kaplan-Meier analysis estimated the probability of total cure at 15 y to be 73.4%. There was no difference in the cure rate between patients with typical and atypical features at one year: 81 and 81.16%, respectively. The probability of cure at 15 y was identical for the two clinical presentations. Conclusions. Pure MVD offers patients affected by Trigeminal Neuralgia due to vascular compression a long-lasting cure in threefourths of the cases. Both typical and atypical presentations respond well to MVD, view in contrast to the classical view that an atypical presentation has an adverse effect on outcome after surgery.

Geç epidural hematomlar da klinik gerilemenin yakın takibi ile tekrar bilgisayarlı tomografi çekilmesi olguların erken saptanmasında en önemli faktördür.

Mitochondria are major targets for ethanol toxicity in the liver, brain, heart, skeletal muscles, and exocrine pancreas (4). Ramachandran et al (23) showed in their study that utilized cultured fetal rat cortical neurons that ethanol elicits a rapid onset of oxidative stress, which culminates in mitochondrial mediated apoptotic cell death. Animal studies have found that long-term ethanol intoxication is not necessary to cause brain █ INTRODUCTION E thanol, is a colorless liquid with the structural formula CH 3 CH 2 OH, often abbreviated as C 2 H 5 OH or C 2 H 6 O. It is also used as a psychoactive drug and is one of the oldest recreational drugs still used by humans. Ethanol can cause alcohol intoxication when consumed. Alcoholic patients can develop not only liver lesions but also pancreatitis, brain damage, peripheral neuropathy, cardiomyopathy, and skeletal muscle myopathy (18,23). Reactive AIm: Ethanol causes oxidative degradation of the mitochondrial genome in the brain. This effect could contribute to the development of brain injury in some alcoholic patients. We investigated the protective effect of caffeic acid phenyl esther (CAPE) and intralipid (IL) on oxidative stress and neurotoxicity induced by ethanol intake. mATERIAl and mEThODS: The forty-eight rats were randomly divided into seven groups. Ethanol was administered for acute toxicity. IL and CAPE were administered immediately after ethanol intake. Total oxidant status (TOS), total antioxidant status (TAS), and oxidative status index (OSi) were evaluated and histologic examination of cerebellum and brain tissue with Hematoxylin-Eosin and immuno-histochemical dyes was performed. RESUlTS: In the ethanol group, TAS levels were significantly lower than the other groups and this finding indicates that the toxic effect of ethanol reduces antioxidant levels. In the ethanol group, TOS levels were significantly higher than the other groups. These results showed that ethanol induced oxidative stress. IL treatment increased TAS levels, and CAPE decreased TOS levels against ethanol toxicity. There was correlation between TAS and TOS levels. Also, histopathologic results confirmed these biochemical results. CONClUSION: CAPE and IL treatment could be effective course of therapy to enhance therapeutic efficacy and may provide a promising approach for the treatment of neurotoxicity and oxidative stress induced by ethanol in clinic.

BACKGROUND: The aim of this study was to investigate the protective effects of erythropoietin, de... more BACKGROUND: The aim of this study was to investigate the protective effects of erythropoietin, dextran/saline and erythropoietin in combination with dextran/saline on brain edema and lipid peroxidation following traumatic brain injury in rats. METHODS: In the study, 40 male 3-month-old albino Wistar rats, weighing 250-340 g, were divided into four groups, each consisting of ten rats. Traumatic brain injury was induced in all rats by the weight-drop method, and erythropoietin (5,000 U/kg) and/or dextran and saline (8 ml/kg) solutions were injected intraperitoneally ten minutes after trauma. Control animals received an equal volume of serum physiologic. All rats were sacrificed 24 hours later. Glutathione peroxidase activity and malondialdehyde levels were measured in the left hemisphere, and edema was quantitated by the wet-dry method. RESULTS: Brain edema and the levels of malondialdehyde, the last product of lipid peroxidation in tissues, were decreased variably, and the activity of glutathione peroxidase, an antioxidant enzyme, was increased in others compared with the control group. CONCLUSION: In this study, it was concluded that the brain edema that developed in rats on which head trauma was induced and the secondary brain damage caused by oxidative stress could be deceased using a combination of erythropoietin, dextran, and saline.

Transverse fractures of the sacrum are even less frequent, and neurological deficit may accompany these fractures. A case of transverse fracture sacrum with cauda equina injury treated lately by sacral laminectomy and S1, S2 root decompression, is reported. Since the first case report of sacral fracture with injury to cauda equine, there have been sporadic reports of such fracture in the literature. We report an unusual case of transverse sacral fracture with neurological damage, which presented to our clinic after four months of injury.

En çok lomber bölgede görülür ve servikal ve torakal bölgede nadirdir. Travma olmaksızın servikal dura içi disk herniasyonu oldukça nadir görülüp, İngilizce literatürde 17 olgu bildirilmiştir ve Brown-Sequard sendromu, transvers myelopati ve radikülopati şeklinde kendini gösterebilir. Biz alt ekstremitede sadece monoplejiye neden olan nadir bir servikal intradural disk hernisi olgusunu sunduk. Bilgimize göre bu literatürde tanımlanan ilk olgudur. Olguya anterior yaklaşımla mikrodisektomi sonrası servikal kafes uygulaması yapıldı.

Neuronavigation provides intraoperative orientation to the surgeon and helps in planning a precis... more Neuronavigation provides intraoperative orientation to the surgeon and helps in planning a precise surgical approach to the targetted lesion and defines the surrounding neurovascular structures. Incorporation of the functional data provided by computed tomography and magnetic resonance imaging or ultrasound with neuronavigation renders neurosergeons to avoid the eloquent areas of the brain during surgery. An intraoperative images enable radical resection of the lesions, the possibility of immediate control for tumor remnants and updates of neuronavigation with these data to compensate for brain shift. This study reviews the pointer and microscope based navigational systems and also highlights the role and to show its indispensibility for a clean surgery with minimal morbidity.

AIM: To investigate whether Nimodipine (N) and Nigella sativa (NS) oil have protective, antioxida... more AIM: To investigate whether Nimodipine (N) and Nigella sativa (NS) oil have protective, antioxidant effects in brain injury caused by experimental head trauma. MATERIAL and METHODS: Fifty albino Wistar rats were randomly divided into 5 groups that underwent experimental head trauma. Oxidative parameters were compared in the serum and brain tissue of the different groups. In addition, apoptosis and caspase-3 immunoreactivity were evaluated by histopathological examination. RESULTS: Serum total antioxidant status (TAS) levels were significantly increased in N and N+NS groups when compared with controls (p=0.001, p<0.01). Tissue TAS levels were significantly higher in the NS and N+NS groups compared to controls (p=0.001, p<0.01). Total oxidant status levels in the brain tissue were significantly higher in the NS group than in the control group (p=0.021). CONCLUSION: N and NS were shown to significantly reduce the occurrence of oxidative stress in secondary brain injury due to head trauma. We also found that apoptosis levels decreased in response to N, NS and N+NS treatments after head trauma.

Initial computed tomography showed the bullet in the right frontal region. In the control scan of the restorative surgery, radiological examination revealed that the bullet had migrated posteriorly due to the effect of gravity, lodging in the entrance hole of posy parietal lobe. Although there are a few literature reports of spontaneous migration of a bullet within the brain, this case was unique as the patient was fully conscious and need to second operation to prevent any neurological deficit

Multiple studies suggest that surgical decompression is an effective therapy for patients with symptomatic lumbar stenosis. Although traditional lumbar decompression is a time honored procedure, minimally invasive procedures are now available which can achieve the goals of decompression with many advantages such as less bleeding, smaller incisions, little muscle distraction and quicker patient recovery. This paper will review the techniques of performing ipsilateral or bilateral decompressions using some microsurgical instruments (microscope, endoscope) and Kit (mild).

Current post-exposure treatments consist of anti-cholinergic drugs and oxime compounds. We studied the effects of intralipid and caffeic acid phenethyl ester (CAPE) on chlorpyriphos toxicity to compose an alternative or supportive treatment for OP poisoning. Forty-nine rats were randomly divided into seven groups. Chlorpyriphos was administered for toxicity. Intralipid (IL) and CAPE administered immediately after chlorpyriphos. Serum acetylcholinesterase (AChE) level, total oxidant status (TOS), total antioxidant response (TAR), and histologic examination of cerebellum and brain tissue with Hematoxylin-Eosin and immunohistochemical dyes were examined. Delayed epidural hematoma is an uncommon finding in patients after intracranial mass evacuation. ... more Delayed epidural hematoma is an uncommon finding in patients after intracranial mass evacuation. It occurs in 6.7-7.4% of cases. A total of 29 reports were found in literature. Among them, no cases of delayed contralateral EDH were due to intracerebral hydatid cyst evacuation. This paper represents a clinical case of a 14-year-old male patient with huge hydatid cyst who underwent left frontopariatel lobe intracerebral hydatid cyst surgery. Next day, a contralateral epidural hematoma was revealed and successfully treated by the conservative treatment via close observation. Contralateral epidural hematomais a life-threatening neurosurgical emergency case, which can occur during first 24 hours after decompressive effects of the cyst removal. Control brain CT scans should be performed next day after the operation to verify and treat these hematomas timely.
