In our study, we compared the results of Farabi Hospital of Black Sea Technical University Medical Faculty, Hacettepe University Medical Faculty Children Hospital and Glasgow Royal Hospital for sick children and tried to show their regional and national differences for antibiotic susceptibility.

The role of quantitative HBsAg in patients with HBV DNA between 2000–20,000 IU/ml

We aimed to determine the contribution of quantitative HBsAg in differentiating chronic infection... more We aimed to determine the contribution of quantitative HBsAg in differentiating chronic infections from chronic hepatitis in HBeAg negative patients with HBV DNA 2000–20,000 IU/ml. A total of 79 untreated HBeAg negative patients were included. Patients were divided into 3 groups based on HBV DNA levels: group 1 (HBV DNA ≤ 2000 IU/ml), group 2 (HBV DNA: 2000–20,000 IU/ml) and group 3 (HBV DNA > 20,000 IU/ml). We collected serum from all patients for quantitative HBsAg analysis. We compared serum quantitative HBsAg levels with biochemical parameters, HBV DNA and liver biopsy results. In this study 46 patients were female and the mean age was 42 years. Serum quantitative HBsAg levels were found to be significantly lower in chronic infections compared with chronic hepatitis. There was a positive correlation between quantitative HBsAg and HBV DNA, ALT (alanine aminotransferase), HAI score (histological activity index), fibrosis score and disease stage. The cut-off level of quantitative HBsAg was determined as 4425 IU/ml to differentiate chronic infection from chronic hepatitis. With the test specificity of 95%, we found quantitative HBsAg cut-off values 1026 IU/ml and 20,346 IU/ml for the diagnosis of chronic infection and chronic hepatitis, respectively. Our study suggests that the quantitative HBsAg ≤ 1000 IU/ml limit value might be used for the diagnosis of chronic infection not only in HBV DNA ≤ 2000 IU/ml but also in patients with HBV DNA between 2000–20,000 IU/ml. In addition, antiviral treatment could be considered in patients with quantitative HBsAg > 20,000 IU/ml and HBV DNA > 2000 IU/ml without further examinations such as liver biopsy.

Bu çal›flmada nozokomiyal Acinetobacter infeksiyonu olan 80 hasta (erkek %70, tüm hastalar için ortalama yafl 38.8 ± 12) de¤erlendirildi. Sceptor sisteminde Enteric MIC/ID ve gram negatif Urine MIC/ID panellerinde (Becton-Dickinson, USA) 53 (%66.3)'ü Acinetobacter baumannii, 27 (%33.7)'si A. lwoffii olarak de¤erlendirildi. Tüm sufllarda ço¤ul antibiyotik direnci vard›. Siprofloksasin ve imipenem direnci, A. baumannii için %60.4 ve %15.5, A. lwoffii için %44.4 ve %22.2 idi. Nozokomiyal infeksiyonlar hastaneye yat›fltan ortalama 14 ± 8.8 günde geliflti. Acinetobacter infeksiyonlar›n›n gösterildi¤i olgular›n 57 (%72.3)'si onkoloji, nefroloji ve yo¤un bak›m servisinde yatmakta idi. Olgular›n 76 (%95)'s›nda ise birden fazla risk faktörü vard›. Nozokomiyal infeksiyonlar s›ras› ile pnömoni (n= 32, %40.8), idrar yolu infeksiyonu (n= 16, %20), yara infeksiyonu (n= 14, %17.5), bakteremi (n= 8, %10), menenjit (n= 5, %6.3), kateter infeksiyonu (n= 3, %3.8) ve peritonit (n= 2, %2.5) idi. Acinetobacter infeksiyonunun neden oldu¤u veya katk›da bulundu¤u ölüm oran› %16.3 idi.

Antibody responses to inactivated SARS‐CoV‐2 vaccine in peritoneal dialysis patients

We aimed to investigate the humoral responses of our peritoneal dialysis (PD) patients to the inactivated SARS‐CoV‐2 vaccine.MethodsHumoral immune responses of 23 PD patients who received two doses of the inactivated SARS‐CoV‐2 vaccine were investigated with a commercial test that measures IgG antibodies towards receptor binding domain of SARS‐CoV‐2 spike protein. Seropositivity rates, antibody titers, and ESRD related clinical data were compared with 51 hemodialysis (HD) patients and 29 healthy volunteers.ResultsSeropositivity of PD patients with the inactivated vaccine was 95.6%. Both the rate of seropositivity and SARS‐CoV‐2 IgG antibody levels in PD patients were not different from the healthy controls (p = 0.85 and 0.19, respectively). While seropositivity rates were not different for PD or HD patients (p = 0.09), the magnitude of humoral responses was significantly higher in PD patients (p = 0.0001). There were no vaccine‐related serious adverse events. In the 3‐months clinical follow‐up, none of the patients experienced SARS‐CoV‐2 infection.ConclusionTwo doses of the inactivated vaccine generate adequate humoral immune response in PD patients without any serious adverse events.

Objective: This study was conducted to describe the demographic, clinical, and microbiological ch... more Objective: This study was conducted to describe the demographic, clinical, and microbiological characteristics of vertebral osteomyelitis in the last decade, mainly by comparing literature and the previous case series performed in our center. Material and Methods: This is a retrospective, observational, descriptive study performed between 2009-2019 at Istanbul University-Cerrahpasa, Cerrahpasa School of Medicine. All patients were divided into three main groups: pyogenic (PVO), tuberculous (TVO) and brucellar (BVO). Results: A total of 100 cases were included in this study. Of these 100 patients, 59 had pyogenic, 15 had brucellar and 26 had tuberculous spondylodiscitis. The disease developed postoperatively in 22 (37.4%) of the 59 PVO cases. The common isolated microorganism was Staphylococcus aureus (n = 11), followed by coagulase negative staphylococci (n = 6). BVO rate was lower than previous case series (15 vs. 24). The median time to improvement in the laboratory findings after the administration of the appropriate treatment was 14 days. PET-CT was diagnostic in 81.8% of PVO patients, similar to MRI. However, PET-CT diagnosis rate was significantly low in TVO patients (3 out of 9, p = 0.040). Conclusion: S. aureus remained the most common etiologic agent. Coagulase negative staphylococci infection rate, mainly related to spinal surgery, and postoperative spondylodiscitis rate is higher than before. Brucellar VO rate is lower, which is mostly related to effective animal vaccination and pasteurization. Although, MRI is the gold standard, PET-CT is a promising technique in diagnosis for PVO.
