This process is associated with organ damage and death. Inducible nitric oxide synthase (iNOS) is increased during hemorrhagic shock and participate in pro-inflammatory signaling. Objectives: The aim of the present study was to examine the protective effects of inhibition of inducible nitric oxide synthase (iNOS) using Amino-guanidine against systemic inflammatory response in a rat model of hemorrhagic shock. Methods: Male Sprague-Dawley rats were assigned to 3 experimental groups (n = 6 per group): Normotensive rats (N); Hemorrhagic shock rats (HS); and Hemorrhagic shock rats treated with AG (HS-AG). After 60 min hemorrhagic shock, rats were treated or not by injection of 1ml of 60 mg/Kg Amino-guanidine (AG) intra-arterially. Rats were then resuscitated in vivo by reinfusion of the shed blood to restore norm tension. The mean arterial blood pressure was monitored. Blood was collected following 60 min hemorrhage and 30 min in vivo treatment and resuscitation for TNF-a measurement. Results: The present study showed that inhibition of inducible nitric oxide synthase (iNOS) using Aminoguanidine decreased the levels of tumor necrosis factor α (TNF-α) in the plasma after one hour of hemorrhagic shock and resuscitation in rats. The results showed that hemorrhagic shock and resuscitation significantly increased the levels of TNF-α. Conclusion: Inducible nitric oxide synthase (iNOS is involved in the up regulation of the inflammatory response in resuscitated hemorrhagic shock. Inhibition of inducible nitric oxide synthase (iNOS) using Amino-guanidine (AG) reduced the inflammatory response to hemorrhagic.

Objective structured clinical examination (OSCE) is one of the types of assessment for undergraduate medical students that has been conducted for first-year medical students in the College of Medicine. Peer student training on participating in OSCE has been conducted prior to the pandemic. The aim of this study was to assess medical students' perception of online virtual peer student video training on participating in the OSCE. Methods: Medical student's council team prepared a video demonstration on the steps of participating in OSCE. In October 2020, the video was provided online for first-year medical students (n = 302) prior to their OSCE. After the completion of the OSCE exam, a 10 – item questionnaire was distributed on the students' perception on the impact of the peer student video demonstration on students' orientation about the OSCE. The questionnaire was sent by Google Forms. The results were expressed as a percentage on a Likert scale. Results: About 74.17% (n = 224) of students responded to the online questionnaire. More than half of the students (n = 126, 56%) found the steps of conduction of the OSCE well explained, provided the sufficient orientation (n = 121, 54%), and found the video stimulating (n = 122, 54%). In addition, the majority of students (n = 160, 71%) learned and understood the steps of the OSCE and 152 students (68%) found that the video helped introducing them to the concept of OSCE. Almost half the students found that the video helped lowering the stress level before the OSCE (n = 91, 41%), eased the steps of the OSCE (n = 113, 50%), and found that the materials were well prepared and explained (n = 126, 56%). Conclusions: The virtual video demonstration of the OSCE during the COVID-19 pandemic had an overall positive perception from first-year medical students. However, peer student's orientation on the OSCE will continue after the pandemic. Further supports for first-year medical students are needed during the pandemic.

Pregraduation online curriculum delivery of internal medicine course during the COVID-19 pandemic: Experience of a medical college in Saudi Arabia

Background: The COVID-19 pandemic has challenged medical education and resulted in a rapid and un... more Background: The COVID-19 pandemic has challenged medical education and resulted in a rapid and unexpected global shift toward online education. The study presents the experience of the College of Medicine of King Saud University in delivering a comprehensive internal medicine course for final-year medical students, which includes the implementation of novel methods of assessment. Methods: A 2-week revision of the internal medicine course was conducted for 70 pregraduation medical students. Lectures and focused case-based tutorials were conducted online. Novel assessment methods included brief written essays summarizing the tutorials, open-book short-answer question (SAQ) examination, and a virtual standardized oral examination (vSOE). A questionnaire was sent to the students (n = 70) on the contents of the course, clarity of objectives, duration, and online platform. Results: The response rate was 99.3%. Students scored higher in the SAQs compared with traditional multiple-choice tests (mean score: 92.3% vs. 69.8%). The vSOE was efficient, and grades were comparable to traditional objective structured clinical examination. The majority of students (93%) reported the scientific content as good or excellent, whereas approximately 30% found the course goals lacked a clear description. The majority (76%) found the course length appropriate, and 94% reported the online platform installation and operation easy or very easy. Conclusions: The comprehensive online internal medicine course was successfully delivered to pregraduation students. Innovative online examination methods can be a reasonable alternative in the case of lockdown. This modality of teaching and assessment was considered useful and met the expectations of students.

Coronavirus disease 2019 (COVID-19) was declared a pandemic by the World Health Organization on M... more Coronavirus disease 2019 (COVID-19) was declared a pandemic by the World Health Organization on March 11, 2020. In response to the pandemic, the Kingdom of Saudi Arabia's Ministry of Education announced the suspension of educational activities. The pandemic has challenged universities worldwide to provide education virtually as an immediate response to prolonged lockdown periods. This article highlights the College of Medicine's experience at King Saud University, Saudi Arabia, in managing undergraduate medical education in response to the pandemic situation. We describe the process of implementing the curriculum's delivery online during the lockdown and the decisions regarding the final assessment for all years. Furthermore, we highlight the steps taken to prepare for the new academic year 2020–2021 as a blended learning approach in light of the COVID-19 situation.

Renal cell injury is caused by tissue ischemia. Renal ischemia initiates a complex and interconnected chain of events resulting in cell injury and renal cell necrosis. Nitric oxide plays a crucial role in renal function and can be inhibited by aminoguanidine (AG). We studied whether AG can ameliorate pathological renal changes associated with HS syndrome in a rat model and explored the AG protection mechanism. Rats were intraperitoneally injected with heparin sodium and mean arterial blood pressure was monitored. Animals were divided into three groups: control (without hemorrhage), with or without intra-arterially injected AG; HS (blood continuously withdrawn or reinfused to maintain an MABP of 35-40 mmHg); and HS with AG. We found that AG decreased plasma concentrations of urea, creatinine, and nitrates; ameliorated histological changes of HS-induced rats; and decreased the expressions of inducible nitrogen oxide synthase (iNOS), proapoptotic protein (BAX), and vitamin D receptors (VDR). AG ameliorated kidney injury by inhibiting iNOS resulting in decreased BAX and VDR expressions. Therefore, a therapeutic strategy targeting AG may provide new insights into kidney injury during severe shock.

Background: International health electives prepare medical students during their student years of... more Background: International health electives prepare medical students during their student years of medical education in choosing, matching, and succeeding in the residency of their choice. They serve as a unique educational experience prior to residency training. Objective: This study was conducted to explore 4th-and 5th-year medical students' perspective about international health electives and to evaluate their educational effects. Methods: An anonymous online survey with 22 items concerning perceptions about the electives with a 5-point Likert scale was administered to medical students at the College of Medicine, King Saud University. Results: For the majority of the students (81.6%) the elective course was arranged by the university and only 18.4% of students individually arranged their own. Most of the students (65.7%) agreed that the objective of the elective course was clear to them in advance. A total of 71.1% of students agreed that the program (elective) was responsive to their needs (both academically and socially). Conclusion: This is the first study looking at international electives in the context of the undergraduate medical curriculum in Saudi Arabia. The study showed that the international electives are a useful and unique experience for medical students (both academically and socially).

We explored medical students' perceptions and opinions about Progress Test (PT) with a view to identifying areas concomitant with it's execution. This cross-sectional study took place at College of Medicine, Saudi Arabia, during the academic year 2015-16. A questionnaire (14 items) was administered. Reason for majority n=96 (89.7%) of the total participants to take the PT was their keenness to compare their academic standing with their peers from other participating medical colleges. The majority of students were highly satisfied with PT implementation; i.e. its orientation (58.9%) and allocated time (90.7%). Students (76.6%) considered PT to offer academic support as future physicians. Students (75.7%) also agreed to participate in the future PT. Students being highly satisfied with the organization of PT. They found it to be a tool helping them to focus on improving the knowledge domain.

Princess Nourah Bint Abdul Rahman University (PNU) College of Medicine is a new medical college established in 2012 that implemented the same innovative reformed curriculum of King Saud University College of Medicine. Objectives: The objective of this study was to use the progress test to evaluate the rate of knowledge acquisition among a new medical school compared to other long-established medical schools in Saudi Arabia. Materials and methods: As part of an ongoing strategy, the progress test was administered before the end of the academic year. Students in PNU were enrolled for 2 years in the progress test. We compared the mean progress test scores for PNU students compared to students at comparable stages in other medical schools in Saudi Arabia. Results: The results showed that the rate of knowledge acquisition was similar in students at PNU to students in other well-established medical schools in Saudi Arabia. Conclusion: The present study showed that the interinstitutional progress test demonstrated that the level of acquisition of knowledge and performance of students in a new medical school was similar to other medical colleges in Saudi Arabia.

This cross sectional study was carried out at King Khalid University Hospital, Riyadh, Saudi Arabia in 2006-2007. One hundred and forty-seven individuals with coronary artery disease (CAD) and 49 healthy individuals matched for age and body mass index were studied. Among CAD patients, 133 underwent angiography. Blood samples were analyzed for total cholesterol (TC), triglycerides (TG), low density lipoprotein (LDL) and high density lipoprotein (HDL) and Lp(a). Coronary artery disease patients had higher Lp(a) levels than controls (25.78 +/- 25.09 mg/dl versus 14.57 +/- 11.81 mg/dl, p=0.0030). Patients without stenosis (10.97 +/- 8.06 mg/dl) and one vessel involvement (19.67 +/- 17.33 mg/dl) had significantly lower levels of Lp(a) compared to double (31.88 +/- 32.17 mg/dl) and triple (29.70 +/- 28.12 mg/dl) vesse...

Background Understanding the reading habits of medical students provide insights and opportunitie... more Background Understanding the reading habits of medical students provide insights and opportunities for medical educators to evaluate the learning needs of the students and improve the teaching I methods. The purpose of the present study was to describe the reading habits of King Saud medical students in a fifth year clinical rotation, which previously have not been described. Methods A questionnaire was administered to 96 medical students rotating through their fifth year at the King Saud University College of Medicine during 2008-2009 academic years. The questionnaire focused on issues related to the reading habits. Results Seventy three of 96 (76%) medical students in a fifth year clinical rotation completed the questionnaire. Students reported reading for an average of 4.3(SD 5) (median three hours per week, range zero to 35 hours per week). The most commonly used and useful reading resource were pocket books and medical text books. Approximately 90% of students reported problems in reading about their patients. Conclusions Students spend sufficient amount of time reading medical pocket books and lecture handouts with less time spend on online sources. The majority of students reported problems in reading that medical educators need to address.
