Global cancer burden reached 19.3 million new cases and 10.0 million deaths worldwide in 2020, resulting in agrowing population of long-term survivors who require structured follow-up and survivorship monitoring. Long-term survival is not uniform across malignancies. SEER registry analysis of more than 4.4 million patientsdemonstrated significantly higher 20-year survival in solid tumours compared with haematological malignancies, withrelative survival improving over time and marked heterogeneity across tumour types such as thyroid cancer andmelanoma. However, survival beyond five years does not eliminate disease risk. Among 62,923 women with estrogen receptor–positive breast cancer who were disease-free after 5 years of endocrine therapy, distant recurrence continued steadilybetween years 5 and 20, with cumulative risks ranging from 10% to 41% depending on initial tumour characteristics. Long-term outcomes also extend beyond cancer recurrence. A meta-analysis of 160 population-based cohort studiesreported a 47% increased risk of cardiovascular disease among cancer survivors compared with non-cancer controls(hazard ratio 1.47; 95% CI 1.33–1.62), indicating persistent excess non-cancer morbidity that contributes to overalloutcome burden. Taken together, long-term outcomes in oncology encompass survival heterogeneity, late recurrence risk, and non-cancermorbidity, underscoring the need for outcome assessment frameworks that extend well beyond the 5-year benchmark. ‘’The information shared on this page is not intended to replace the advice of a doctor or pharmacist. One often overlooked consequence is hearing loss. For some patients, treatment-related hearing impairment can affect communication, learning, social participation, and daily functioning long after therapy has ended. High-risk and rare diseases often challenge one of healthcare’s most critical assumptions: that identifying the“right patient” is straightforward. In reality, patient selection is rarely defined by a single marker or diagnosis. Biology may guide the first step, but meaningful decisions often require a broader view, combining clinical phenotype,progression risk, comorbidities, treatment history, and the realities of day-to-day practice. Two patients with the samediagnosis can follow very different paths, with very different needs. This is especially relevant in complex and underserved disease areas, where time to intervention, access to expertise, andindividualized assessment can significantly influence outcomes. As precision medicine evolves, so must our understanding of precision itself.
