Critical Analysis of Radiologist-Patient Interaction' 565 A critical incident interview technique... more Critical Analysis of Radiologist-Patient Interaction' 565 A critical incident interview technique was used to identify features of radiologist-patient interactions considered effective and ineffective by patients. During structured interviews with 35 radiology patients and five patients' parents, three general categories of physician behavior were described: attention to patient comfort, explanation of procedure and results, and interpersonal sensitivity. The findings indicated that patients are sensitive to physicians' interpersonal styles and that they want physicians to explain procedures and results in an understandable manner and to monitor their well-being during procedures. The sample size of the study is small; thus further confirmation is needed. However, the implications for training residents and practicing radiologists in these behaviors are important in the current competitive medical milieu.

Open-ended interviews with 107 patients documented specific patient expectations of radiologic pr... more Open-ended interviews with 107 patients documented specific patient expectations of radiologic procedures during which there was no direct radiologist-patient interaction. Patient expectations could be classified into those related to the facility and those related to interactions with radiology staff. Among facility-related expectations, waiting time far outweighed all other concerns. Interpersonal skills were the predominant expectation of radiology staff. The role of the radiologist in fulfilling patient expectations was less clear. Only 10% of unprompted patients cited the radiologist as a factor in their expectations. When patients were specifically prompted to discuss the radiologist's role, communication skills, accuracy of interpretation, and interpersonal skills were the predominant concerns.

Six challenges to meaning are identified and described: physiological changes, disruption of norms, overreliance on social network, stressful autonomy, psychosocial vulnerability, and biographical disruption. Each of these challenges has the potential to erode the three domains of meaning in life, coherence, purpose, and mattering.

Picture Archiving and Communication Systems (PACS) represent an enormously expensive technologica... more Picture Archiving and Communication Systems (PACS) represent an enormously expensive technological innovation which has the potential to alter the way in which radiology is practiced. In a 1985 survey of radiologists and radiology administrators, over a quarter of those individu1s expected to have some component of an operational PACS system by 1986. The survey respondants perceived advantages including fast access to files, multiple user simultaneous access to films, and possibly secondary benefits in accuracy in diagnosis and improved communications. Progress in widespread PACS implementation has considerably lagged the rate anticipated in the 1985 study. In order to evaluate the current expectations of PACS and contrast them to the earlier enthusiasm we have undertaken this study of the current perceptions of PACS among both radiologists and clinicians.

However, meaning can be affected by a variety of events. Trauma of many types causes a necessary reevaluation of meaning, and perhaps a reconstruction. Similarly, life transitions may have a deleterious influence on meaning. This chapter considers contemporary models of meaning in life and examines the effect of trauma on meaning and the potential effect of life transitions.

This study emphasizes the importance of development of communication skills and communication standards, with particular emphasis on written data as the single most important factor in keeping a strong clinician referral base.
