🧪Introducing quantitative total testosterone testing as the newest addition to our point-of-care assay menu. Back in the spring, we explored here how stress, sleep and wider health factors can influence testosterone levels, while sharing we were “doubling down” on testosterone and to “watch this space”. Since then, there has been a steady drumbeat of news around testosterone, with it moving firmly into the public-health conversation. 🔊 Earlier this month, research presented at ESHRE found significant declines in men’s total testosterone, free testosterone and SHBG between 1972 and 2019. 1️⃣The Guardian reported an estimated 54% reduction in average total testosterone across that period. 🚩Rising obesity and diabetes are likely to account for part of the change, while environmental and lifestyle factors remain under investigation. It's clear that testosterone cannot be understood through symptoms alone. 💤Fatigue, reduced libido, erectile dysfunction, mood changes and loss of physical capacity have many possible causes. A testosterone test result provides useful biochemical context, but it still needs to be considered alongside the patient’s history, sampling time, medication, body composition and wider health. 🩸Using a small capillary blood sample, the assay provides a result in approximately 15 minutes, allowing the result and the appropriate next step to be discussed while the patient is still present. That next step may be: repeat morning measurement, additional hormonal or metabolic testing, referral, or monitoring under clinical supervision. A rapid result does not remove the need for interpretation. It makes that interpretation available sooner. This week, Implantologie Journal have announced the publication of brand-new findings that question what else may be relevant before implant, augmentation, or periodontal treatment begins. 📖 'Vitamin D3: Erfahrungswerte aus einem Jahr PoC-Testungen' by Dr. Bastian Wessing and Hannah Thomas (Implantologie Journal 5/26, pages 44-47). Based on 132 Vitamin D chairside measurements (results within <15 minutes) in an implant-surgical, augmentative, and periodontal setting over 12-months, 75% of patients did not meet the >30 ng/mL Vitamin D sufficiency threshold. With growing research surrounding the impact Vitamin D deficiency may have on implant outcomes, these new data represent a real-world signal that a potentially relevant, modifiable factor can remain invisible. Rather than an argument for blanket supplementation, the growing body of evidence is an important reference for clinicians aiming to optimize discussions surrounding healing, osseointegration, and treatment readiness.
