With rising rates of diabetes, hypertension, and obesity, we cannot continue responding only when patients reach kidney failure. The future of nephrology must extend beyond the clinic. While regional strategies recognise the importance of tackling Non-Communicable Diseases (NCDs), kidney health still requires greater policy focus and coordinated action. Here are four challenges that deserve urgent attention: 📊 The Data Gap Without comprehensive renal registries and consistent CKD surveillance, policymakers are often required to make decisions using incomplete data. Better data leads to better planning. 💰 The Financial Burden Kidney replacement therapies place substantial pressure on both healthcare systems and patients. Expanding access to early detection and appropriate treatment options can improve outcomes while supporting more sustainable healthcare delivery. 🏥 Too Much Reactive Care Far too many people are diagnosed only after significant kidney damage has already occurred. Earlier screening through routine eGFR and urine albumin testing offers an opportunity to intervene before progression to kidney failure. 🌍 Regional Risk Factors Matter Climate, infectious diseases, genetics, diabetes and hypertension all influence kidney health across the Caribbean. Effective CKD policies should reflect these regional realities rather than relying on a one-size-fits-all approach. As healthcare systems around the world strengthen their response to Chronic Kidney Disease (CKD), one question continues to challenge me: Are we measuring success by the number of patients we identify—or by the number of lives we improve? Growing up in, and working closely with, the Caribbean healthcare landscape has highlighted the realities of delivering kidney care in resource-constrained settings. In many countries, the absence of national renal registries, comprehensive policy frameworks, and sustainable funding continues to contribute to fragmented care and significant out-of-pocket costs for patients. Naturally, we often look to established healthcare systems such as the UK’s NHS for inspiration. Through national strategies, NICE guidance and primary care quality improvement initiatives, the NHS has significantly improved the identification of people living with CKD. However, experience also highlights several important lessons for healthcare systems worldwide: 🛑 The Screening–Treatment Gap Identifying CKD is only the first step. Many eligible patients still do not receive optimal evidence-based cardio-renal therapies, including SGLT2 inhibitors. 👩⚕️ Primary Care Capacity As clinical guidelines become more comprehensive, primary care teams need the workforce, education and resources to translate policy into better patient outcomes. 📱 Digital Health Must Remain Human-Centred Technology can transform healthcare, but it should support—not replace—compassionate clinical communication. Patients deserve understanding, reassurance and shared decision-making alongside digital innovation. A Lesson for Global Health Policy As Caribbean nations continue strengthening kidney care strategies, we have an opportunity not simply to replicate existing models, but to build healthcare systems that combine early detection, integrated cardio-renal-metabolic (CRM) care, robust health data and patient-centred implementation. Success should not be measured solely by the number of patients entered into a registry, but by the number of patients whose lives are improved. Healthcare is about people. The first dialysis patient probably felt one overwhelming emotion: Hope. Dialysis was a medical breakthrough. It transformed kidney failure from a fatal diagnosis into a condition that could be treated. It gave patients another chance at life. Today’s patients still value dialysis—it continues to save countless lives every day. But many also ask a different question: “Doctor… I wish my kidney disease had been found earlier.” In the decades between those two moments, medicine has advanced remarkably. We now understand kidney disease better than ever before. We have earlier diagnostic tools, better blood pressure management, improved diabetes care, kidney-protective medications, digital health technologies, and AI-supported risk stratification. Yet too many people still begin dialysis having learned about their kidney disease only when it is already advanced. Perhaps the next chapter in nephrology is not about building more dialysis units. Across the Caribbean, individuals of African and Afro-Caribbean ancestry face a disproportionately higher risk of progressive kidney disease due to a combination of genetic, clinical, and environmental factors. 🧬 APOL1 Genetic Variants Certain APOL1 variants, more commonly found in people with West African ancestry, are associated with an increased risk of chronic kidney disease. While these variants do not determine a person’s future, they highlight the importance of targeted screening and early intervention. 🩸 Sickle Cell Disease & Kidney Health Sickle cell disease—and in some cases sickle cell trait—can silently affect kidney function long before symptoms appear. Early recognition and regular monitoring can help preserve kidney health. As a Consultant Nephrologist, I believe the future of kidney care lies in precision prevention—identifying those at highest risk before irreversible kidney damage occurs. Many people with chronic kidney disease don’t reach a nephrologist until their kidneys have already suffered irreversible damage. By then, our options are often more limited than any of us would like. Those experiences have influenced how I think about kidney care. I believe we need to spend as much energy on preventing kidney failure as we do on treating it. Earlier diagnosis, better use of data, stronger primary care pathways and appropriate use of digital technology all have an important role to play. That’s one of the reasons I started Safesoul Ltd—to explore practical ways that clinicians and technology can work together to support earlier intervention and better patient pathways. Recently, I’ve been developing ideas around how these approaches might support kidney care in the Caribbean, alongside healthcare professionals, policymakers and local stakeholders. It’s still a journey of learning, collaboration and refinement, but I believe there is real opportunity to strengthen prevention and improve outcomes. For kidney transplantation, geography often becomes one of the biggest barriers—not the surgery itself. The economics are worth considering. Current approach: Move the patient ✈️ Air ambulance transfer: US$8,000–50,000+ per mission 🚁 Helicopter transfer: US$5,000–20,000+ 🏥 Additional costs: * Medical escort teams * Hospital admission and stabilization * Ground ambulance transfers * Accommodation for patients and families * Delays that may affect organ viability A single inter-island transfer can easily exceed US$20,000–60,000 once all associated costs are included. A future possibility: Move the kidney Imagine a regional network using: * Advanced organ preservation * AI-enabled cold-chain monitoring * Autonomous drones for inter-island transport A dedicated medical drone mission could potentially cost hundreds to a few thousand US dollars, depending on the aircraft, distance, infrastructure, and regulatory framework. While organ cryopreservation and long-distance drone transport of donor kidneys are still under development and not yet routine clinical practice, the economic potential is compelling. Beyond cost savings ✅ Faster organ delivery ✅ Reduced cold ischaemia time ✅ Better donor–recipient matching ✅ Fewer discarded donor kidneys ✅ Less need for expensive patient transfers ✅ Improved access to transplantation for smaller island nations The bigger question If we can reduce transport costs by 80–95%, improve transplant logistics, and potentially save more lives… Shouldn’t we start designing the future today? The Caribbean has a unique opportunity to become a global leader in smart transplant logistics, combining transplantation, AI, organ preservation, and autonomous aviation to overcome the challenges of geography. Back then, most doctors had one career: Doctor. You did that for 30 or 40 years, perhaps in different roles, but medicine was your career. Period. Today, through Moneywise Doctor, podcast conversations and teaching thousands of doctors, I see something very different. Many doctors are no longer just doctors. Not that there's anything wrong with being a career doctor. More and more, I meet... Doctor + Author Doctor + Coach Doctor + Founder Doctor + Educator Doctor + Expert Witness Doctor + Content Creator Doctor + HealthTech Adviser Some have quietly built businesses that even many of their colleagues know nothing about. This episode focused on drowning prevention which is an important public health issue that disproportionately affects children and adolescents around the world. This show is timely as schools are on summer break and many people are enjoying our beautiful beaches. It was an engaging and informative discussion with an outstanding panel: - Dr Monnique-Chadee Waite, Head of the Emergency Department - Dr. Sani Amatti, Deputy Chief of Medical Services and Head of Paediatrics & Neonatology - Oshin Whyte, Environmental Scientist and Divemaster Together, we discussed hospital emergency department statistics, practical water safety tips, and resources to help build water competency. What made the session even more meaningful was the incredible engagement from our FaceBook community. The questions, comments, and recommendations shared will help shape our ongoing drowning prevention awareness strategy. The two photos tell a story. The first was taken an hour before we went live. There I am, reading through my script for what felt like the hundredth time. Yes, it's about being prepared but it's also how I calm my nerves. No matter how many speeches I give or shows I host, the butterflies never really go away. That little voice still whispers, "Don't mess up." The perfectionist in me is always present. The second photo captures the smiles after the show. I'm grateful to our panelists for sharing their expertise, grateful for the community's participation, and, if I'm honest, also grateful that the show was over! Now, don't get me wrong, I genuinely love doing this, which is why I keep saying yes. For those who have known me over the years, this journey has been significant. I was the introvert. The shy girl. The one who preferred to observe rather than speak. The "nerd" who was often seen but rarely heard. The workshop brought together health professionals from across the Caribbean to strengthen skills in electronic medical certification of cause of death (eMCCD), ICD-11 mortality coding, validation, and the benefits of high-quality mortality data for decision-making. Officers from the Ministries of Health and other government agencies across ten (10) Caribbean countries - including Aruba, Antigua and Barbuda, Barbados, the British Virgin Islands, Dominica, Grenada, Saint Kitts and Nevis, Saint Lucia, Saint Vincent and the Grenadines and Trinidad and Tobago, benefitted from training in the use of the new DHIS2 Mortality Module, ICD -11 Cause of Death Application and other software tools used in coding and analyzing mortality data, including DORIS, Code Edit and ANAcoD3. Bulk of the kidney disease in diabetic patient are associated with nephropathy complications. Hypertension is usually found in chronic type 1 and type 2 diabetes sufferers. Numerous factors like vascular remodelling, basement membrane thickening, vascular atherosclerosis, mesangial cell proliferation, oxidative stress in renal apparatus due to ischemia etc. due to both the hypertension and hyperglycemia leads to expansion and evolution of nephropathy in these patients. Management of hypertension is therefore highly advisable in diabetes patient as an important intervention to prevent vascular damages like nephropathy. The aim of hypertensive drug management in diabetic patient should include both the management of blood pressure as well as albuminuria management. ACE inhibitors are found to be the most appropriate drugs which complies to both these requirement while the use of other antihypertensive drugs require caution due to their derogatory influences on the glucose and lipid metabolism in diabetic patients.
