  • CMS's proposed 2027 Medicare Physician Fee Schedule has sparked an important industry conversation around the future of Remote Patient Monitoring (RPM). While the proposal is still under review, it reinforces the importance of clinically integrated, physician-directed care and raises important questions about how to preserve access, affordability, and scalability as healthcare continues to evolve. At Brook, we believe RPM is an important capability — but it's only one part of a broader continuous care model. Improving outcomes requires more than monitoring devices; it requires patient engagement, AI, clinical expertise, and coordinated provider workflows that support patients between visits.     📊 Nearly three-quarters of patients receiving consistent monitoring and Brook nurse-led care management achieved hypertension control within 12 weeks. That’s one of the key findings from our recently published JMIR study — and it highlights something healthcare is increasingly recognizing: Chronic care outcomes are determined between visits, not just during them. The study found that patients receiving adequate RPM monitoring alongside Brook’s nurse-led care management demonstrated significantly better hypertension control outcomes, including a 27% higher prevalence of control at 12 weeks compared to less-monitored patients. What’s driving the difference isn’t just more data collection. It’s continuous engagement, proactive clinical oversight, and intelligent workflows operating together as one coordinated system.
