About us
Center for Elders’ Independence PACE offers comprehensive community-based medical and social care all in one bundle. We serve East Bay seniors who are over 55 years of age and who who need nursing home care - but we keep them living at home! CEI personal care teams coordinate and integrate whatever care a senior participant needs - home care, activities, transportation or medical care - everything from a hospital stay to an Oakland A's Game with other participants. Though CEI does not provide housing, we coordinate with senior housing, residential care facilities, hospitals and the family caregivers through any circumstances. Services offered: • Primary Care/Nursing Care • Hospital Services • Emergency Services • Nutrition and Hot Meals • Social Services • Home Health Care • Recreational & Social Activities • PACE Day Center • Medications • Transportation • Medical Specialists (cardiologists, podiatrists, etc.) • Rehabilitation Therapies • Dental Care PACE is associated with lower nursing home visits and admissions, fewer hospitalizations, a higher probability of receiving ambulatory care, greater survival, and increased number of days in the community, better health, better quality of life and better functional status. Caring for someone you love can be rewarding and exhausting. If you’re feeling constantly tired, anxious, irritable, losing interest, or having trouble sleeping, you might be heading toward caregiver burnout. It affects your health, relationships, and daily life. Take short, guilt-free breaks: step outside, read, or sip a cup of tea, to recharge and protect your ability to give great care. Center for Elders' Independence reposted this
We were grateful to host this month’s Senior Care Sales Collective at our Center for Elders' Independence Concord location! A huge thank you to the team at HALLWAY HEALTHCARE, as well as Kiara Wade and Delinda Brown, for their coordination. We were also privileged to welcome Julie Derby Jaecksch, an End-of-Life Doula. Julie shared her expertise on how she walks alongside individuals and families to bring comfort, dignity, and peace to the final stages of life. Thank you to everyone who joined us to connect, collaborate, tour our center, and participate in these essential conversations! #finalseasondoula,#CenterForEldersIndependence #ConcordCA #SeniorCareSalesCollective #EndOfLifeDoula #SeniorCare #ElderCare

Community is at the heart of everything we do at Center for Elders’ Independence. This past week, our Outreach & Enrollment team had the opportunity to volunteer with the Alameda County Community Food Bank, spending the day sorting and organizing fresh produce alongside other incredible organizations dedicated to serving our neighbors. Together, our collective efforts helped contribute to more than 60,000 meals for individuals and families throughout Alameda County. Giving back is one of the most meaningful ways we can strengthen the communities we call home. Whether we’re connecting older adults to the care they deserve or volunteering our time, we believe that small acts of service create a ripple effect of kindness, hope, and connection. We are grateful to be part of a community that shows up for one another and proud to stand beside so many volunteers making a real difference. Here’s to continuing to lift each other up, pay it forward, and build a healthier, stronger community together. 💜 #CenterForEldersIndependence #CommunityMatters #GiveBack #Volunteer #AlamedaCounty #CommunityService #PayItForward

We are honored to be asked to help tell the story of the impact our care has for elders when they encounter life changes. Thank you CalPACE!
"We will not leave you." Those four words capture what person-centered care should look like. As California's population ages, more families are asking an important question: How can we help older adults remain connected to their communities while receiving the care they need? This thoughtful article from News & Review, features Center for Elders' Independence and explores the importance of honoring dignity, independence, and choice as we rethink how we support older adults. It's a conversation worth having, and one that reflects the values at the heart of the PACE model. Read the full story: https://lnkd.in/eY5rikUw #CalPACE #PACE #HealthyAging #AgingWell #CommunityCare

Soccer fever is sweeping the Bay Area, and CEI participants are getting in on the action! Participants recently attended an Oakland Roots Sports Club match, enjoying an afternoon of excitement, community spirit, and hometown pride as anticipation builds for the FIFA World Cup games happening in the region. At PACE, staying active and connected is just as important as medical care. Outings like these help participants build friendships, stay engaged, and experience the joy of being part of their community. #OaklandRoots #SoccerFever #WorldCup2026 #PACE #CenterForEldersIndependence #HealthyAging #CommunityConnection
