This piece reinforces what we’ve been saying all along: In CNS, operational architecture is what determines whether a safety signal gets caught or missed. As CNS gene therapies move from single-center proof-of-concept to multicenter trials, they generate safety data that standard eClinical monitoring frameworks weren't built to detect. The challenges are distinctly CNS: • Direct intracerebral delivery means neurological adverse events present differently across sites, depending on surgical experience, imaging capability, and post-op monitoring intervals • Standard RBM platforms (calibrated for oncology or metabolic disease) carry risk thresholds and escalation workflows tuned to the wrong safety phenotype • Validated motor instruments like the MDS-UPDRS were designed for chronic drug treatment, not the overlapping effects of surgical trauma, immune response, and transgene expression in the first 90 days post-injection • AAV follow-up expectations stretch 2–5 years, demanding longitudinal monitoring long after the surgical team has moved on So, we'll put the author's implied question to this community: As CNS gene therapy scales, who should own solving the monitoring gap, sponsors, CROs, or eClinical vendors?
