Today is a win for patient access. The Pharmacy Compounding Advisory Committee’s recommendation that six out of seven peptides receive Category 1 inclusion is an important step toward preserving a regulated pathway for licensed providers to prescribe personalized compounding medications for patients whose care does not fit a standardized protocol. Peptides. GLP-1s. Compounding regulation. Shaun Noorian sat down with Jeffrey Gladden, MD, FACC to talk through the issues shaping patient access to personalized medicine right now.

Drug shortages are not rare exceptions. They are a recurring reality, and compounding pharmacies exist to solve them for the patients caught in the middle. As written, the SAFE Drugs Act would cap certain compounded medications at 20 units per month and restrict compounding tied to the FDA shortage list, regardless of clinical need. It would also require FDA inspection of 503B outsourcing facilities before production can begin, delaying the very supply this bill claims to protect. For patients, that means fewer options when a commercial drug is unavailable, discontinued, or wrong for their needs. For pharmacists, it means less flexibility to act on clinical judgment for a patient depending on them. Arbitrary caps and pre-production delays don't improve safety. They slow down access when patients can least afford to wait. The Senate HELP Committee may mark up this bill on July 22.
