Sometimes improving health starts with a conversation. Through iMPROve Health's Social Isolation & Loneliness (SIL) Pharmacy Screening Program, pharmacists helped older adults who were feeling isolated connect with local support services. One referral helped an older woman reconnect with her community, improve her nutrition, and regain a sense of belonging. This initiative demonstrates how healthcare providers can address social needs and improve well-being by seeing the whole person, not just their medical conditions.
