Mount Darwin, Zimbabwe. Summer temperatures above 40°C. Aqueline is nine months pregnant. Daily life involves caring for her family and working in the fields. For decades, guidance on safe heat exposure was built around studies of healthy young men – not pregnant women facing extreme heat. That oversight has real-world consequences, especially in a warming world. The odds of preterm birth increase by 26% during heatwaves, and Mount Darwin already experiences several each year. As part of the HAPI project (Heat Adaptation for Pregnant Women and Infants), researchers and community health workers are developing practical solutions. These include cooling kits containing a sunhat, water bottle and fan, homes with reflective white roofs, and daily heat alerts delivered through an app called Mother Heat Alert. "We're about changing lives with very simple solutions that can be scaled," says Fortunate Machingura, PhD, who leads the project. "Working with communities is at the core of our work." Wellcome has invested £20 million in research on heat and pregnancy across four continents – building the evidence needed to make protecting pregnant women and newborns from extreme heat routine.
