If you work in revenue cycle for an inpatient rehab facility, you know the frustration: two patients with similar diagnoses can generate very different payments — and it's not always clear why. The answer usually comes down to how CMS's IRF-PPS methodology stacks adjustments on top of a base rate: 🧾 CMG assignment — Driven by the IRF-PAI assessment: motor/cognitive function, Rehabilitation Impairment Category, and age all determine which Case-Mix Group a patient falls into. 🧠 Comorbidity tiers — Qualifying secondary conditions bump payment up through Tier 1, 2, or 3 — accurate coding here has a direct dollar impact. ➗ The core formula — CMG relative weight × the annual standard payment amount = base payment. 📅 FY2026 updates — CMS raised the standard payment amount to $19,371 (up from $18,907 in FY2025), a net 2.6% increase after productivity adjustments. 🏢 Facility-level factors — Wage index, rural adjustment, Low-Income Percentage, teaching status, and outlier payments all move the final number further. ⏱️ Short-stay rules — Stays under 3 days get routed to a special short-stay CMG, and early transfers get prorated — regardless of what the "expected" CMG payment would've been. Every one of these variables updates annually, which means pricing logic that isn't current is pricing incorrectly.
