  • Brain fog, fatigue, hair shedding... they can all be driven by low iron. But to know what’s really going on, you need to test more than just your iron levels. Ferritin is an acute phase reactant — meaning it rises with inflammation, independent of your actual iron status. A ferritin of 60 ng/mL in a patient with elevated inflammatory markers means something very different than a ferritin of 60 ng/mL in a patient without. When interpreted in isolation, that number can mask a real deficiency. This is where the "right" ferritin target gets contested. Conventional labs flag deficiency below 12–15 ng/mL. Functional medicine often targets 50–70 ng/mL for symptom resolution. Recent research on non-anemic iron deficiency suggests the diagnostic threshold itself should move to 30–50 ng/mL to better catch women who are iron deficient without being anemic. And here’s the thing: none of these numbers are wrong — they're just answering different clinical questions. The conventional range answers: when is someone at risk of iron deficiency anemia? The functional range answers: when is ferritin low enough to plausibly explain fatigue, hair shedding, or poor recovery in a patient whose bloodwork otherwise looks unremarkable? A complete iron picture means looking at ferritin alongside serum iron, TIBC, transferrin saturation, and inflammatory markers like hs-CRP. This is the actual work of practicing evidence-based functional medicine at scale: not choosing the most aggressive number and applying it universally, but building protocols around the full clinical picture, and revising them as the research evolves.
