We've been managing Medicare Advantage populations for years now, and this tracks with what we've seen on the ground: chronic conditions with high variability in outcomes are precisely where whole-person, evidence-based care shows its value. The #IBD data point is a good example of a broader pattern - chronic disease prevalence in aging MA populations keeps climbing, and it's rarely confined to the conditions or age groups people expect. Programs that were designed around "young adult disease" assumptions need to evolve alongside the population. The push toward whole-person, evidence-based, outcomes-driven care isn't just a payer trend anymore, it's showing up in federal policy too. CMS's new MAHA ELEVATE model, for example, is putting real funding behind whole-person approaches to chronic disease in Medicare. It's worth noting who that Medicare population increasingly includes: CDC data show IBD prevalence among Medicare fee-for-service beneficiaries 67+ has risen roughly 3% a year for two straight decades. Research also estimates elderly-onset cases now account for 10 to 20% of new diagnoses nationally. Two independent, peer-reviewed studies previously presented at Digestive Disease Week found the same thing: IBD patients enrolled in a proactive, between-visit care coordination model had significantly fewer ED visits and hospitalizations than risk-matched peers. Outcomes-based care only works if incentives are actually aligned for everyone in the room: patients need to stay healthier and avoid the ER; specialists need workflow that doesn't add administrative burden, plus a way to be rewarded for better outcomes; payers need real, measurable cost reduction they can point to. IBD patients cost health plans more than 3x what non-IBD members cost: $22,987 vs. $6,956 per member per year in one national dataset. Nearly $9.3B is spent annually on IBD treatment in the U.S., roughly half of it on hospital care. Here's the part that should stop payers and providers alike: the majority of IBD patients who end up hospitalized hadn't seen a doctor in the 30 days before admission. That's not a treatment failure. It's a visibility failure. IBD patients don't usually know they're getting sicker until it's an emergency. Symptoms creep. Patients adjust. "This is just my normal" becomes the baseline, until a flare turns into an ER visit, a hospitalization, or a surgery that could have been avoided with an earlier signal. That's the pattern behind every "high beta condition": high cost, high variability, and complications driven by symptom intensification patients don't recognize in themselves.
