I opened up a dispenser cabinet on a job and found this. A 400HS-30 water/particulate filter, dripping at the base. This is exactly the kind of thing that gets missed. Dispenser cabinets are closed boxes, and busy operators running multiple sites can't be opening every panel on a normal day. A slow drip doesn't shut anything down. It doesn't trip an alarm.
