𝐅𝐚𝐬𝐭𝐞𝐫 𝐚𝐜𝐜𝐞𝐬𝐬 𝐭𝐨 𝐦𝐞𝐝𝐢𝐜𝐚𝐥 𝐢𝐦𝐚𝐠𝐞𝐬, 𝐦𝐨𝐫𝐞 𝐭𝐢𝐦𝐞 𝐟𝐨𝐫 𝐩𝐚𝐭𝐢𝐞𝐧𝐭 𝐜𝐚𝐫𝐞 At Bernhoven, patients are encouraged to remain as much in control of their treatment journey as possible. To support this approach, the hospital recently took an important step by implementing 𝗘𝗻𝗼𝘃𝗮𝘁𝗶𝗼𝗻’𝘀 𝗫𝗗𝗦 𝗩𝗶𝗲𝘄𝗲𝗿 for the secure and efficient exchange of medical images. With the volume of diagnostic data continuing to grow, 𝐁𝐞𝐫𝐧𝐡𝐨𝐯𝐞𝐧 was looking for a solution that was dependable, user-friendly and straightforward to deploy. The project came with an additional challenge: the hospital was also introducing a new EPR at the same time. Thanks to close collaboration between all parties involved and a flexible implementation approach, the XDS Viewer was successfully tested and rolled out in 𝐣𝐮𝐬𝐭 𝐭𝐰𝐨 𝐦𝐨𝐧𝐭𝐡𝐬. We’ve successfully delivered a major upgrade to our Electronic Patient Record (EPR) system and started the phased rollout of AI-powered Ambient Voice Technology (AVT). Together, these improvements will help clinicians access information more quickly, streamline care, and reduce administrative tasks, allowing more time to be spent with patients. The EPR upgrade introduces digital clinical documentation for inpatients and extends electronic prescribing, as well as digital imaging requests and reporting across outpatients. We’re also introducing AVT following a successful pilot and testing phase. The technology securely creates draft clinical notes from consultations, which clinicians review and approve before being added to the patient record and shared with patients and Gps. Reflecting on the achievement, Group Chief Digital Information Officer Will Monaghan said: “Our ambition is to become a fully digitally enabled Trust, improving the quality of care and the experience of care for patients. We also want to make UHL an even better place to work by giving colleagues digital tools that genuinely help. I want to thank everyone involved for their commitment to making UHL a better place to receive and deliver care.”

I recently completed my first white paper alongside the development of a supporting prototype and published both on Zenodo. The inspiration: Hospital readmission remains a critical issue as it is often used as an indicator of the quality of patient care. Patients leave the hospital hoping to resume their normal routine or establish a new normal but many unfortunately require readmission. An important distinction, however, must be made between avoidable and unavoidable readmissions. This paper discusses both, while focusing on how avoidable readmissions may be reduced through better discharge planning, monitoring, and decision support. The paper explores the causes and impacts of hospital readmissions, together with the role of the Hospital Readmissions Reduction Program (HRRP) in encouraging improvements in patient outcomes. The primary objective was to develop An Adaptive Hospital Readmission Framework, a decision-support framework designed to assist clinicians, administrators, and care teams during discharge planning while also providing operational oversight for monitoring performance and identifying opportunities for improvement. Alongside the framework, I developed a prototype that demonstrates how the concepts presented in the paper can be translated into a practical support tool. This paper introduces the framework.
