DICOM traffic is not “just another app flow.” A slow or unavailable path stalls radiology workflows. The job here isn’t just keeping a VIP alive — it’s keeping studies moving. Most teams stop at TCP reachability. The bigger issue is operational resilience, tuned specifically for imaging workloads: - Routing has to handle bursty image retrieval without becoming the bottleneck. - Health checks need to do more than confirm a port is open; a generic TCP check does not tell you whether the PACS service is actually responding. - Failover has to complete before a stalled connection turns into a stalled read. Loadbalancer.org supports Layer 4 DR mode for DICOM, plus a native DICOM ECHO health check — which is the difference between pinging a port and validating the imaging service properly. If you support PACS, VNA, or other imaging systems, design for throughput, health validation, and low-latency failover from day one. Retrofit it later, and a stalled study stops being an IT problem and starts becoming an operational one. #HealthIT #PACS #LoadBalancing

🚨 Many PACS demos look perfect in conference rooms. The real test begins after deployment. In actual hospitals and diagnostic centres, a PACS has to deal with: 🌐 Slow or unreliable internet connections 🔄 Upload interruptions and retries 📦 CT, MRI, and PET-CT studies containing thousands of images 🗂️ Vendor-specific DICOM quirks and inconsistent metadata 📑 Duplicate or incomplete series 🏥 Multiple hospitals and diagnostic centres sending studies simultaneously 💾 Storage, memory, and network bottlenecks None of these problems show up during a product demo. They show up when an emergency CT is being uploaded over an unstable connection, multiple centres are sending studies at the same time, and a radiologist is waiting for the images to load.
