When discussing fraud, waste, and abuse (FWA), organizations often focus on detection and recovery. But what if prevention started much earlier? Operational workflows, program design, organizational change management, and technology decisions all influence program integrity long before issues surface. Complex environments call for upstream solutions, as Kim Bazan identifies here: https://lnkd.in/ghs8_X4e #ProgramIntegrity #FraudPrevention #PublicSector #GovernmentTechnology #HumanServices #FWA

Owner and executive-level fraud often takes longer to detect. Why? People in senior positions may control information, influence employees, approve transactions, or override established procedures. Their authority can discourage others from raising concerns. The nonprofit findings in the ACFE 2026 Report to the Nations reveal that owner-level fraud lasted a median of 24 months without fraud awareness training, compared with 15 months where training existed. The report also shows that owners and executives accounted for 16% of perpetrators in the 2026 study, compared with 12% in the ACFE’s first report three decades earlier. Trust is important, but trust without oversight can create opportunity. No employee should be above accountability. No executive should be beyond independent review. No unusual transaction should escape appropriate scrutiny. Strengthen board oversight, management accountability, reporting systems, and investigation capabilities. Fraud risk is not always obvious at the point a decision is being made. A supplier email may look legitimate. A business contact may appear credible. A claim, transaction, online profile or company background may seem routine at first glance. The difficulty for many organisations is that warning signs are often spread across different public sources — company records, online footprints, media reports, social platforms, domain information, scam indicators and open web material. Individually, these details may not mean much. Structured properly, they can help form a clearer risk picture. ClearSignal Intelligence is being developed to support professional organisations with lawful public-source intelligence, enhanced due diligence, fraud-risk awareness and litigation support. The focus is simple: Clear, ethical and professionally presented intelligence that helps organisations identify red flags, understand risk and make better decisions. If your organisation deals with fraud risk, due diligence, disputes, claims, financial crime or corporate risk, this is the space we are building in.

Risk has a cost. So does litigation. So does uncertainty. Recently, USAA announced nearly $1 billion in combined savings and returns to eligible Florida members, citing reduced legal costs and improved market conditions following Florida's tort reform efforts. Regardless of where you stand on tort reform, the announcement highlights an important reality: Risk management decisions often have financial consequences far beyond the original security concern. Insurance costs. Claims. Litigation expenses. Operational impacts. Reputation. Organizations that proactively identify and address vulnerabilities are often better positioned to reduce both risk exposure and the costs associated with it. At NGA Security Advisors, our work in CPTED, security consulting, and risk assessment is focused on helping clients understand those vulnerabilities before they become incidents. Because effective security isn't just about protection. It's also about reducing risk and improving resilience. 🎆𝗧𝗵𝗶𝘀 𝗜𝗻𝗱𝗲𝗽𝗲𝗻𝗱𝗲𝗻𝗰𝗲 𝗗𝗮𝘆, 𝗟𝗲𝘁'𝘀 𝗖𝗲𝗹𝗲𝗯𝗿𝗮𝘁𝗲 𝗔𝗰𝗰𝗼𝘂𝗻𝘁𝗮𝗯𝗶𝗹𝗶𝘁𝘆 𝗮𝗻𝗱 𝗜𝗻𝘁𝗲𝗴𝗿𝗶𝘁𝘆 As we celebrate the freedoms and opportunities that define our nation, it's also a good time to reflect on the importance of accountability, one of the foundations of public trust. Whether in government, nonprofits, tribal organizations, or the private sector, fraud undermines the very principles that organizations work hard to uphold. Strong internal controls, ethical leadership, and a culture of transparency are essential to safeguarding resources and maintaining confidence among stakeholders. The most effective fraud prevention efforts don't begin with investigations; they begin with prevention: ✅ Setting the right tone at the top ✅ Encouraging employees to speak up ✅ Monitoring for risks and anomalies ✅ Strengthening internal controls ✅ Ensuring accountability at every level This Fourth of July, I'm reminded that integrity is not just a compliance requirement, it's a commitment to those we serve, the responsible stewardship of public resources, and the courage to remain independent from those who seek to misuse, manipulate, or defraud the systems we are entrusted to protect. If your organization is facing fraud risks, compliance concerns, control weaknesses, or simply wants to strengthen its fraud prevention framework, CLA (CliftonLarsonAllen) forensic, risk, and internal controls professionals are here to help. Don't wait for a red flag to become a crisis. Proactive prevention is one of the best investments an organization can make. Wishing everyone a safe, meaningful, and happy Independence Day. #FourthOfJuly #FraudPrevention #FraudRiskManagement #Ethics #Integrity #InternalControls #Accountability #ForensicAccounting #Governance #PublicTrust #CLAConnects #CLAForensics

How much fraud risk is sitting in your alert backlog right now? For many insurers, the challenge is no longer generating fraud alerts. It is working through them quickly enough for investigators to act while opportunities are still there. What stood out to me is that backlog is more than an operational issue. It quietly affects investigation quality, resolution speed and ultimately the value your fraud detection capability delivers. Worth a read if you're wrestling with growing alert volumes, limited investigation capacity or prioritisation challenges. Particularly relevant for SIU leaders and claims operations teams.
