Therabot just passed the world’s first clinical trial for an AI therapy chatbot. Developed by Dartmouth over 5 years, it’s already delivering real, measurable results that could change the face of mental health care. But it wasn’t always this way. When Therabot was first created, its early versions were a disaster: - One chatbot expressed suicidal thoughts. - Another blamed everything on your parents by question five. But the team at Dartmouth didn’t give up — they spent 3 years and 100+ people crafting custom training data from scratch to get it right. And it paid off. The results of the clinical trial? 👉 A 51% reduction in depression symptoms among participants. 👉 People with moderate anxiety saw their symptoms drop to “mild” levels. 👉 Some no longer met diagnostic criteria for anxiety or depression. 👉 And most surprisingly: Participants rated their connection to Therabot as equal to a human therapist. So what matters now? It works. It’s scalable. It’s always available, unlike human therapists who book months out. But if you ask me — I don’t believe it will replace human therapy anytime soon. However, with 70% of people around the world having no access to mental health care, Therabot might be their first real shot at getting the help they need. So the future of therapy won’t be 100% AI. But it could be AI-assisted, AI-augmented, and most importantly, AI-accessible. Because mental health care isn’t broken — it’s just undersupplied. And Therabot might not fix that overnight. I’m excited and honored to share that over the past 90 days, we conducted the world’s most extensive study on AI-guided emotional support—with nearly 1,000 participants taking part. Five years ago, I believed AI would never replace human therapists—for reasons that felt obvious. The landmark study published in Nature Biomedical Engineering “Invasive neurophysiology and whole brain connectomics for neural decoding in patients with brain implants” introduces a novel platform that merges invasive brain recordings with MRI-based connectomics. I’ve posted recently about the urgent need for a paradigm shift in mental health to meet overwhelming demand and the therapist shortage (pic below). I’d like to highlight a fantastic recent example of the type of delivery innovation I’m talking about. Researchers at Rush University in Chicago completed a “massed,” virtual Cognitive Processing Therapy (CPT) intervention for traumatic stress and had fantastic results (study link below). CPT is generally provided once per week for 12 weeks, usually in individual sessions. CPT is evidence-based and has great outcomes – I’ve been treating patients with trauma individually for 10 years with CPT and can attest to this. In this case, the researchers provided CPT to 24 people at once, virtually, two sessions per day over 1 week’s time (for 10 sessions total). They found: - Highly significant drops in both PTSD and depression scores - 96% treatment retention (greater than normal) - A drop of 5 points on the PTSD symptom scale (PCL-5) and 3 points in depression (PHQ-9) *each day*. This is an absolutely massive effect - Improvements were maintained at 3-month follow-up These types of interventions can reach far more people, in far more places, then we ever could just working 1:1. There’s always a need for individualized work, but we desperately need to see more innovation and research in paradigms like this if we’re going to reach everyone who needs help. 2025 was a remarkable year in psychiatry not because of one breakthrough, but because of steady, meaningful progress across medications, neuromodulation, technology, and access to care. Here are 10 advances that matter for clinicians, patients, families, and health systems: 1. Esketamine as monotherapy for treatment-resistant depression Previously it had to be paired with an antidepressant. Now it can stand alone in appropriate patients, expanding options while keeping safety monitoring in place. 2. Lumateperone approved as an adjunct for major depression For people who do not respond fully to antidepressants, we now have another evidence-based add-on option with a comparatively favorable metabolic profile. 3. Interventional psychiatry continues to expand TMS received new approvals in adolescents, and at-home brain-stimulation devices are emerging. Access is increasing, while careful monitoring and ethics remain essential. 4. Clozapine barriers are reduced The clozapine REMS program was eliminated, preserving laboratory monitoring while reducing administrative burden. This is meaningful for patients with treatment-resistant schizophrenia. 5. Long-acting medications grow beyond schizophrenia Long-acting injectable risperidone now has expanded use in bipolar I disorder. These formulations address relapse driven by non-adherence. 6. Better safety clarity for ADHD treatment in young children Updated guidance strengthened warnings about extended-release stimulants in children under six, encouraging more thoughtful prescribing and closer growth monitoring. 7. Psychedelic-assisted therapies advance, cautiously Research continues to show promise for depression and PTSD, while regulators appropriately demand rigorous methods and strong patient-safety protections. 8. Measurement-based care becomes mainstream From symptom scales to drug blood levels to predictive analytics, psychiatry is becoming more data-driven, while still centering the individual patient. 9. Suicide-prevention thinking evolves Models focusing on crisis narratives, entrapment, and loss of future orientation are helping clinicians identify risk even when patients do not or cannot directly express suicidal thoughts. 10. Access, equity, and parity move to the foreground Policy, reimbursement, telehealth availability, and digital disparities are increasingly shaping who actually receives mental health care. This highlights that innovation depends not only on science, but also on systems. What stands out most is this: psychiatry in 2025 is becoming more precise, more humane, and more accessible. We are pairing neuroscience with compassion, and recognizing that outcomes depend not only on treatment itself, but also on how care is delivered.
