After years of coaching students through the ADC Part 1, we've noticed that almost every student moves through three distinct stages during their preparation. Knowing which stage you're in changes how you respond to it: 😰 Stage 1: The overwhelm stage This is the beginning. The syllabus feels enormous. The references feel dense. Herpes Zoster (Shingles) Cause: Reactivation of VZV Who: Older / immunocompromised patients Features: Pain 3–5 days before rash → unilateral vesicles on erythematous base → never crosses midline → follows trigeminal dermatome Oral: Vesicles → ulcers (maxillary or mandibular) Complications: Vision loss / Ramsay Hunt syndrome / post-herpetic neuralgia Management: Antivirals within 72 hours (all patients) Urgent referral: Eye / CNS / disseminated involvement 3. Hand, Foot and Mouth Disease (HFMD) Cause: Coxsackievirus / Enteroviruses Who: Children — highly contagious Features: Fever + malaise + sore throat → vesicles ulcerate → painful oral ulcers + skin lesions on hands, feet, around mouth Management: Supportive (hydration + analgesics); self-limiting ~3 weeks 4.
