To submit MSHA forms online you must be registered with MSHA EGov and Login.gov. To create accounts and submit MSHA forms online follow below steps:

Create MSHA EGov Account:

Create a Login.gov Account:

Note: Your Login.gov account email address must be the same email address registered with MSHA EGov.

After registering with MSHA EGov and Login.gov navigate to Forms Online Filing.

On the Forms & Online Filing webpage, locate desired form, select online filing option (red computer icon) in the form row,
select Login.gov button, enter your Login.gov credentials (email address, password, and two-factor authentication). Complete the form and submit to MSHA.
Note: Ensure you are using one of the supported web browsers either Microsoft EDGE or Google Chrome. Firefox, Safari, and Internet Explorer are not supported browsers for MSHA EGov online filing.
