Aim To determine which of the two methods, Kegel exercises or combination of Kegel exercises with... more Aim To determine which of the two methods, Kegel exercises or combination of Kegel exercises with the use of the KegelSmart biofeedback device, has better therapeutic effects on the symptoms of SUI in females. Methods Fifty female patients with SUI were randomly divided into two groups: 25 treated with Kegel exercises, and 25 with the combination of Kegel exercises with the use of the KegelSmart biofeedback device. Patients in both groups performed Kegel exercises 30 minutes daily for 30 days. Patients in the second group, in addition to Kegel exercises, applied the KegelSmart device intravaginally for 20 minutes daily for 30 days. All patients filled out a questionnaire based on 12 questions consisting of an objective and a subjective component. Results The basic characteristics of the patients from both groups were not statistically significantly different: age 55.16 vs 54.52 years; number of births 1.80 vs 1.96; body mass index 29.12 vs 28.40. There was a statistically significant reduction in the values of all analysed objective and subjective parameters in the group treated with combination of Kegel exercises with the use of the KegelSmart biofeedback device compared to Kegel exercises group. Conclusion Combination of Kegel exercises with the use of the KegelSmart biofeedback device has better therapeutic effects than Kegel exercises on the objective and subjective symptoms of SUI.

Lumbar spinal stenosis is the leading cause of spinal surgery in adults older than 65 years and impairs quality of life, in terms of reduced activity and limitations that affect psychosocial prosperity 2. Patients with an indication for operative treatment are clear case. But in their daily work, the dilemma is the treatment of those who are not candidates for surgical treatment or who refuse surgical treatment. Conservative treatment is aimed at alleviating the major clinical manifestations of degenerative instability and makes the patient long-lasting relief. Conservative treatment involves active physical therapy, conducting physical training at home, postural education and non-steroidal anti-inflammatory drugs 3. Non-surgical treatment is almost always recommended as initial treatment 4. The aim of this study was to evaluate whether physical therapy can lead to significant improvements in patients with lumbar spinal stenosis.

Determination of changes in the cervical spine of patients with rheumatoid arthritis (RA) is impo... more Determination of changes in the cervical spine of patients with rheumatoid arthritis (RA) is important. Although they are often clinically asymptomatic, during exercise therapy complications, or even death, may occur. The aim of the study was to determine the factors which indicate changes in the cervical spine and atlantoaxial joint (AA) in patients with RA, as well as the association between those changes and changes occurring on the hands. The study included 80 patients with RA who were divided into two groups according to the duration of the disease (up to 10 years and more than 10 years). Structural changes in the hands and cervical spine were monitored by ordinary radiography. Structural changes in the cervical spine were found in both groups of patients without a statistically significant difference between them (p = 0.165). The AA joint was more often deformed in patients with a longer duration of the disease (p = 0.012). The changes on the hands were worse in patients who had the disease for longer than 10 years (p = 0.002), and they correlated with AA subluxation (p = 0.002) and luxation (p = 0.004), as well as with an erosion of the cervical s pine (p = 0.000). According to our findings, in order to recognize changes in the cervical spine, patients with RA must be regularly monitored radiographically, and monitoring should be mandatory in patients who had a longer duration of the disease as well as those with more advanced structural changes on the hands.

In order to preserve functional ability of patients with rheumatoid arthritis (RA) one needs to c... more In order to preserve functional ability of patients with rheumatoid arthritis (RA) one needs to continuously apply exercises which will preserve range of joint motion. There is not enough data in the literature about their real value in the preservation of joint movement of RA patients. The aim of this work was to examine the effects of home exercising on the joint movement duration one year, as well as to determine optimal treatment periods of RA patients in hospital conditions. WORK METHODS: Examination was conducted on 31 RA patients treated at the Clinic for physical medicine and rehabilitation. During their treatment patients were educated about their illness and they were trained to exercise, independently, at their homes every day for 30 minutes in order to improve the range of motion. Their functional disability was assessed by Health Assessment Questionnaire (HAQ) that was conducted three times; at the end of first rehabilitation period, and then after a year at the beginning and the end of new rehabilitation period at the Clinic. Results: Most examined patients (55%) did not have any education, and poor economic condition of life had 69% of the patients. All examined patients have been exercising at home, alone, although most of them, as a result of exposed deformities had a need for assisted exercising. Among them 56.1% of the patients exercised every day and 43.9% exercised occasionally (2-3 times a week). Mean HAQ value was 1.25 after the end of first rehabilitation period. Achieved results were preserved during the following 8 months, but after that period HAQ increased to 1.90 (by 96.7% of the patients). After new rehabilitation treatment, which lasted for four weeks, HAQ returned to its earlier level (1.26). During the examined period there was no significant difference between the HAQ among patients that exercised on a daily basis and the ones that exercised occasionally (p > 0.05). Discussion: Low level of education and poor socio-economic conditions influenced the decreased effect of education. Only 7% of the patients visit the rheumatologist for control every 1-2 months. According to Ward study, less than 7 visits to rheumatologist per year is connected with the progression of functional disability. Patients with University degree had the least decrease in HAQ after a year. After new rehabilitation HAQ has been improved by all patients, and it has been returned to earlier value in 80.35% of the patients. Conclusion: Home exercises are not sufficient in order to preserve functional ability of RA patients. Examination determined that rehabilitation needs to be conducted in the specialized institutions at least once a year, optimally twice a year. It contributes to reduction of progression of functional disability.

In 43 patients, 39 women and 4 men, with SLE (disease was diagnosed according to revised American College of Rheumatology ACR criterias, 1997), aged between 20 and 61 and averaged disease duration of 5.54 +/- 5.74 years, heart/lung radiology and heart echosonography were performed in order to discover possible serositis (pericarditis and exudative pleuritis) prior to and after cytotoxic and glucocorticoid therapy. The presence of pericarditis and exudative pleuritis was established in 20 patients (47%) before the therapy. After the therapy pericarditis was present in 2 patients, average volume of 150 ml, and exudative pleuritis was also present in phrenicocostal sinus, but its volume was minimal. Systemic corticosteroids (CS) are often used in treatment of Crohn's disease and are usually ... more Systemic corticosteroids (CS) are often used in treatment of Crohn's disease and are usually considered the main cause of osteoporosis in patients with Crohn's disease. The aim of this paper was to assess the influence of CS on development of osteoporosis in Crohn's disease. In this study bone mass density (BMD) was analyzed in 43 patients with Crohn's disease. Most of the examined patients (72.1%) used CS for more than 3 months. Osteoporosis was diagnosed in 30.2% of patients, and osteopenia in 44.2% of patients. There were no statistically significant difference in BMD among patients who were on CS and the ones who were not. In the group of newly diagnosed patients 60% had reduced bone density, opposing 61.6% of patients with previously diagnosed disease. Patients with rheumatoid arthritis (RA) and osteoarthritis (OA) suffer from a lot of pain, have d... more Patients with rheumatoid arthritis (RA) and osteoarthritis (OA) suffer from a lot of pain, have difficulties with movement, so it is therefore logical for them to have less social contact, i.e. socializing with friends and family. Aim of the study was to examine social relations of the patients with RA and OA, and to establish the reasons for such relations. Survey conducted 55 patients, 29 patients with RA and 26 patients with knee OA. From all patients data from domain of social contacts have been taken. They filled in self-evaluation scale for depression by Zung-SDS, and functional ability was assessed by Health Assessment Questionnaire HAQ (for RA) and by Laquesne index (for OA). Most patients in both groups narrowed the number of people with whom they socialize 82.8% RA patients and 80.7% OA patients and now they socialize with the closest family. The aim of this study was to evaluate the presence of atherosclerotic plaque of head and neck blo... more The aim of this study was to evaluate the presence of atherosclerotic plaque of head and neck blood vessels and to determine the dynamics of circulation through the brain blood vessels in patients with systemic lupus erythematosus (SLE). In 35 patients with SLE aged 37.67+/-9.96 and whose disease lasted 3.8+/-4.51 years, Doppler carotid ultrasonography was used to identify the presence of intima-media thickness or atherosclerotic plaque. Brain perfusion scintigraphy was done in 15 out of 35 patients in order to evaluate the dynamics in circulation through carotid and cerebral media arteries. Measured by Doppler ultrasound, 2/35 of examined lupus patients had a plaque and the 2/35 had an intimal-medial thickness. The results of perfusion scintigraphy in 15 examined patients out 35 with SLE showed that 5/15 had mild circulatory changes in carotidogram. 4/15 patients had mild changes in cerebra media arteries circulation, 1/15 had severe changes in carotid circulation and 5/15 patients...

Aim of the study was to examine the possibilities of physical treatment of patients with the most severe degree of structural changes caused by knee osteoarthritis. Examination was conducted on 60 patients that were on physical treatment because of the knee OA. Structural changes are evaluated by Kellgren-Lawrence scale, functional ability by Lequesne index, and pain by Visual analog scale. Physical treatment lasted for 4 weeks. After the physical treatment there was a significant improvement of functional ability (p = 1.78E-07), but the size of improvement was reduced by the level of structural changes. It was significantly lower in IV class in relation to III and II class (p < 0.05). Rheumatoid arthritis is a chronic, progressive, systemic disease where most visible changes are i... more Rheumatoid arthritis is a chronic, progressive, systemic disease where most visible changes are in the joints that lead to the damage, which ultimately results in loss of function. In order to reduce damage to the joints it is necessary to diagnose early, before the appearance of erosion and to include adequate therapy. The problem is the lack of criteria for diagnosing the beginning of disease. Modified classification criteria of the American College of Rheumatology are not sensitive for the diagnosis at an early stage. It takes great knowledge and experience of rheumatologist to determine the existence of inflammatory arthritis, to exclude other possible arthritis, to determine the risk of developing persistent, erosive arthritis and that in time that is designated as a “window of opportunity” propose an optimal therapeutic strategy. EULAR has given recommendations for managing early rheumatoid arthritis.

Objective-To determine the nutritional status of children and adolescents with cerebral palsy (CP... more Objective-To determine the nutritional status of children and adolescents with cerebral palsy (CP) in relation to the degree of functional disability. Patients and methods-This cross sectional study analyzed the nutritional status of 73 children and adolescents of both sexes suffering from CP, with an average age expressed as a median of 9.6 years (range 2.6 to 19.4). Evaluation of nutritional status in patients with CP was done on the basis of Body Mass Index (BMI). The degree of functional disability was assessed on the basis of the Gross Motor Function Classification System (GMFCS levels I-V). BMI percentiles were expressed by sex and chronological age, based on national standards in 2000 Centers for Disease Control and Prevention Growth Charts for the United States. Results-Of the total number of participants, 21.9% were underweight (BMI <5 th percentile), 11% were overweight (BMI from 85 th to 95 th percentile) and 9.6% were obese (BMI >95 th percentile). Most malnourished participants had a clinical form of quadriplegia, whereas and overweight and obese had diplegia. There is a significant negative correlation between BMI percentile and the degree of functional disability according to the GMFCS (p=0.0001). Immobile participants (GMFCS IV-V) showed a high prevalence of malnutrition (39.4%) compared to the self-mobile participants (GMFCS I-II, 4.3%). Obesity was present in 17.3% selfmoving participants (GMFCS I-II) and in 17.6% mobile participants with mobile device support (GMFCS III), and none of the immobile children were obese. Conclusion-About 40% of children and adolescents with CP have nutrition problems, where half of them are malnourished and the other half are over-nourished. Immobile children and those with quadriplegia have a higher risk for malnutrition, whereas mobile children and those with diplegia have a higher risk for over-nutritional status.
