Clinical Profile and Admission Outcomes of Patients With Acute Pulmonary Embolism at a Resource-Constrained Hospital in Ghana: A Retrospective Pilot Study

There is a paucity of literature regarding this condition from the Ghanaian perspective. This study aimed at evaluating the clinical profile and admission outcomes of patients with acute pulmonary embolism at the Methodist Hospital, Wenchi, Ghana. Methods: A retrospective pilot study was conducted by analyzing medical records of patients diagnosed with acute pulmonary embolism from September 2023 to March 2025. Data on demographics, clinical manifestations, risk factors, investigations, treatment, and outcomes of admission were collected. Results: Out of 17 patients enrolled in the study, 11 (64.7%) were females. The mean age was 58.8 ± 13.0 years. The commonest risk factors were age > 65 years (n = 6), immobilization (n = 5), obesity (n = 4), and recent surgery (n = 3). Hypertension and diabetes mellitus were the predominant comorbidities. Dyspnea (76.5%, n = 13) and chest pain (n = 5) were the most frequent presenting symptoms while tachypnea (70.6%, n = 12), tachycardia (70.6%, n = 12), and hypoxia (52.9%, n = 9) were the commonest clinical signs. Three patients had hemodynamic instability. Pretest probability assessment was infrequently performed (n = 7). Sinus tachycardia (76.5%, n = 13) was the predominant electrocardiographic abnormality. The majority (58.8%, n = 10) had intermediate-risk pulmonary embolism whereas 3 patients were classified as having high-risk pulmonary embolism. Two in-hospital mortalities were recorded. Conclusion: Clinical features and predisposing factors were largely similar to those described in the literature. Also, pretest probability assessment was underutilized. In-hospital mortality rate (11.8%) was relatively high. Large-scale, multicenter prospective studies are recommended to assess long-term complications of acute pulmonary embolism such as recurrence and chronic thromboembolic pulmonary hypertension. Skin Hyperpigmentation as Initial Manifestation of Vitamin B 12 Defciency: A Case of Delayed Diagnosis With Late-Onset Subacute Combined Degeneration of the Cord

Rarely, skin hyperpigmentation may be the sole initial manifestation of this micronutrient defciency, which can easily be overlooked, leading to delayed diagnosis. This case report describes a 32-year-old Ghanaian male who presented with a 4-year history of progressive darkening of the dorsal aspects of the interphalangeal joints of both hands, the palms, and soles of the feet, for which he did not seek immediate medical attention. He later developed a worsening unsteady gait. Neurological assessment revealed mild weakness in both lower limbs, brisk knee refexes, diminished ankle refexes, and bilateral extensor plantar refexes. Additionally, he had decreased sensitivity to fne touch, impaired proprioception, and diminished vibration sense. His gait was ataxic with a positive Romberg's sign. Laboratory investigations showed macrocytosis with anemia and a markedly reduced serum vitamin B 12 level. Upper gastrointestinal endoscopy with biopsy identifed chronic gastritis. He was diagnosed with vitamin B 12 defciency secondary to chronic gastritis complicated by subacute combined degeneration of the cord. Following parenteral repletion of vitamin B 12 , the skin hyperpigmentation completely resolved. His neurological symptoms also signifcantly improved. This case report highlights the need to recognize skin hyperpigmentation as a potential initial symptom of cobalamin defciency, which is critical for early diagnosis and prompt initiation of treatment to prevent debilitating neurological complications.

It affects 2-3% of the general population. It often runs a benign course and the majority of patients are asymptomatic. Cardiac sources of thrombi account for about 20% of ischaemic strokes. There is lingering uncertainty regarding the causal link between mitral valve prolapse and ischaemic stroke. It has been suggested that changes on the surface of the valvular leaflets trigger platelet adherence and aggregation, leading to formation of platelet-fibrin thrombus which can embolize to the cerebral arteries, causing an ischaemic stroke. We herein report a case of a 25 year old young man who presented with sudden onset of left-sided weakness. Cardiac auscultation was notable for a late systolic murmur preceded by a midsystolic click at the apex. Transthoracic echocardiography showed an elongated and thickened anterior mitral valve leaflet which prolapsed into the left atrium during systole. Computed tomography scan of the brain revealed infarcts in the right frontal lobe and the right insular cortex. A diagnosis of myxomatous mitral valve prolapse complicated by ischaemic stroke was made and the patient was accordingly started on anticoagulant therapy with warfarin.

Occasionally, affected individuals may experience acute respiratory distress and retrosternal pain which can mimic lifethreatening conditions like pulmonary embolism and myocardial infarction. This case report underscores the need for clinicians to be aware of this rare syndrome as it is amenable to conservative treatment in most instances.

Background and Aims: Urinary tract infections are among the most common infections globally, with... more Background and Aims: Urinary tract infections are among the most common infections globally, with increasing rates of antibiotic resistance complicating their management. This study aimed to determine the prevalence, bacteriological profile, and antibiotic susceptibility patterns of uropathogens isolated from urine samples at Methodist Hospital, Wenchi, Ghana. Methods: A retrospective study was conducted using data on urine culture and antibiotic susceptibility testing obtained from the Microbiology Unit of the Laboratory Department between March 2024 and January 2025. Results: A total of 504 urine samples were analyzed, revealing a prevalence of urinary tract infections at 45.2%. The predominant pathogens were Gram-negative bacteria, with Klebsiella spp (12.3%) and Escherichia coli (10.1%) being the most frequently isolated organisms. Candida spp were also identified in 5.8% of the samples. Females accounted for 74.6% of the infections, with the highest prevalence observed among individuals aged 20-40 years. Antimicrobial susceptibility testing demonstrated significant resistance among bacterial isolates, with Klebsiella spp showing the highest resistance to Beta-lactams and Fluoroquinolones. Amikacin exhibited the lowest resistance rate (4.4%), making it a strong candidate for empirical therapy. Multi-drug resistance (MDR) was demonstrated by 72.7% of bacterial isolates, with Klebsiella spp and Escherichia coli showing the highest MDR rates. Conclusion: The identified patterns of antimicrobial resistance highlight the critical importance of careful antibiotic selection to effectively manage urinary tract infections. Poor Glycemic Control and Its Predictors Among Type 2 Diabetes Patients: Insights From a Single-Centre Retrospective Study in Ghana

Background and Aims: The primary objective of glycemic control in individuals with diabetes melli... more Background and Aims: The primary objective of glycemic control in individuals with diabetes mellitus is to avert or postpone complications, which ultimately leads to an improved quality of life. Nonetheless, achieving the recommended targets for glycemic control in clinical settings often proves challenging. Consequently, it is crucial to ascertain factors that affect glycemic outcomes to enhance the management of diabetes mellitus. This study sought to evaluate the levels of glycemic control and the associated factors among patients with type 2 diabetes receiving care at the Methodist Hospital, Wenchi, Ghana. Methods: A retrospective study was conducted using an existing database. Glycemic control was evaluated by HbA1c measurements with a target of < 7% indicating good control, as per the guidelines established by the American Diabetes Association for nonpregnant adults. HbA1c levels ≥ 7% were classified as poor control. Data analysis was conducted using SPSS version 25 and multivariate logistic regression analysis was employed to determine the factors affecting glycemic control. Results: The median HbA1c level among the participants was 7.9% (IQR: 5.8-9.9). Majority (59.3%) demonstrated poor glycemic control with HbA1c ≥ 7%. Factors associated with poor glycemic control included advanced age (AOR: 4.32, 95% CI: 0.61-11.21, p = 0.012), duration of diabetes mellitus > 10 years (AOR: 3.60, 95% CI: 1.05-9.82, p = 0.019), insulin therapy (AOR: 3.13, 95% CI: 0.55-11.01, p = 0.009) and hypertension diagnosis (AOR: 2.88, 95% CI: 0.75-5.45, p = 0.030). Conclusions: The study indicated that a considerable proportion of individuals with diabetes exhibited inadequate glycemic control. Older age, longer duration of diabetes mellitus, insulin therapy and comorbid hypertension were significantly associated with poor glycemic control among the study population. Multidisciplinary interventions as well as customized management strategies are required to ensure effective glycemic control to prevent long-term complications of diabetes mellitus. 1 | Introduction Diabetes mellitus is a metabolic disorder marked by persistent hyperglycemia which arises from inadequate utilization of glucose for energy and its excessive production due to dysregulated glucose metabolism including gluconeogenesis and glycogenolysis [1]. Esophagogastroduodenoscopy, also referred to as upper gastrointestinal endoscopy, is an important and safe procedure used for both diagnosis and treatment in the field of gastroenterology. This study sought to assess the characteristic clinical indications and endoscopic findings of individuals presenting at Methodist Hospital, Wenchi, Ghana. Methods: A retrospective analysis was conducted by examining the medical records of patients who underwent upper gastrointestinal endoscopy from April 2023 to June 2024. Results: Out of 405 patients who underwent upper gastrointestinal endoscopy during the study period, 396 were included in the subsequent analysis. The average age of patients was 43.7 years with a standard deviation of 16.6 and ranging from 12 to 100 years. Approximately 42.9% of the patients were males. Dyspepsia was the most common reason for upper gastrointestinal endoscopy (60.9%), followed by retrosternal chest pain (17.7%), heartburn (12.1%) and upper gastrointestinal bleeding (3.8%). Results of upper gastrointestinal endoscopy were normal in 15.2% of patients. The most prevalent pathologies of the esophagus, stomach and duodenum were gastroesophageal reflux disease (17.4%), gastritis (31.8%), and duodenitis (4.5%), respectively. Conclusion: Dyspepsia was the primary indication for endoscopic evaluation of the upper gastrointestinal tract. The most prevalent pathological finding was gastritis, followed by gastroesophageal reflux disease, gastric ulcer, hiatal hernia and duodenitis. Esophagogastroduodenoscopy is an essential investigative tool for diagnosing upper gastrointestinal tract pathologies. Chilaiditi's Sign: A Rare Finding in a Patient with Concurrent Liver Cirrhosis and Chronic Lymphocytic Leukemia

Chilaiditi's sign is a rare radiological finding characterized by colonic interposition between t... more Chilaiditi's sign is a rare radiological finding characterized by colonic interposition between the liver and the right hemidiaphragm. It is a cause of pseudopneumoperitoneum which is often mistaken for true pneumoperitoneum. In this article, we present the case of a 73-year-old female with a previous history of pulmonary tuberculosis, who reported to our hospital with progressive abdominal distension associated with early satiety, bipedal swelling, generalized weakness, worsening fatigue, and weight loss. Following evaluation, she was diagnosed with liver cirrhosis and chronic lymphocytic leukemia. Her chest radiograph also incidentally showed air under the right hemidiaphragm with haustral folds consistent with Chilaiditi's sign. Since she did not have signs of peritonitis, she was eventually treated for liver cirrhosis and chronic lymphocytic leukemia without undergoing any surgical intervention. This case report seeks to create awareness among clinicians about this rare radiological sign in order to prevent misdiagnosis leading to unnecessary surgical procedures.
