The NHS spent £242 billion in 2024/25, a figure that has been rising for a decade. Yet more money has not necessarily translated into more and better care. The question is no longer how much the NHS spends. It is how the money already in the system gets used. 📄 The NHS has published plenty of quality frameworks. The new 10-year Quality Strategy for NHS-funded care, published on 14 July, tries something different. Responding to the Darzi and Dash reviews, it makes quality the organising principle for all NHS-funded care, built on one value-based definition rather than new targets. That definition spans three domains (safety, effectiveness and experience) and is delivered through seven initial priorities and ten enablers, with a strengthened National Quality Board to hold it together. But with no new targets and no significant new money, the strategy lives or dies on whether those enablers, and the data behind them, actually land. That makes the practical starting point clear: build the quality-management and data capability the strategy assumes, so you can act when priorities and metrics arrive rather than scramble to measure them. 💡 In 2024/25, failing NHS buildings caused more than 4,100 disruptions to patient care: cancelled appointments, delayed treatment, care interrupted by the estate itself. The NHS’s 10 Year Capital Plan, published by the Department of Health and Social Care on 8 July 2026, sets out to change that. It commits to: 🔹 Repair ageing hospitals and remove unsafe crumbling concrete to make buildings safe 🔹 Progress the New Hospital Programme 🔹 Join up patient care through a single patient record and an expanded NHS App. 🔹 Digitise care itself, with AI to support clinicians and new hospitals and health centres built digitally enabled. 🔹 Preventing illness through research funding, genomics, and investment in pandemic preparedness and biosecurity But the biggest shift is not what gets built. It's the move from one-year settlements to long-term certainty, with faster approvals and more control handed to local NHS organisations. The open question now is deliverability: whether the ambition on paper becomes reality on the ground. 🏥 Defining the right care pathways is only half the job. If the way a health system pays for care does not reinforce those pathways, little changes in practice. Payment reform is one of the most powerful levers available, but it works best when it is focused, tied to clear outcomes and supported by the right governance. Saudi Arabia is not starting from scratch here. The establishment of the Center for National Health Insurance, the consolidation of providers under the Health Holding Company, and the maturing of health clusters are all creating the conditions for a genuine shift in how care is commissioned and paid for. 🫀Heart disease and stroke still cause around 33,000 early deaths a year in England. Most are preventable. The new cardiovascular disease modern service framework, published this week by the Department of Health and Social Care and NHS England, sets out to change that with a single ambition: to cut premature deaths by a quarter in a decade, and a shift from reactive hospital care to prevention in neighbourhoods. The framework does 3 things: 🔹 Reorganises cardiovascular care around a core principle 🔹 Highlights 12 priorities where the evidence is strongest and delivery today is most inconsistent 🔹 Holds integrated care boards (ICBs) and local government accountable for outcomes The ambition is clear. The harder question is delivery: turning separate condition registers into a single view of risk and reaching the people currently missing from services. After a wonderfully busy start to the year, the CF team spent the end of last week together on the sunny coast of Eastbourne for our summer retreat. ☀️ This retreat had a competitive edge: a tough team quiz on night one, and a CF sports day the following afternoon to test everyone's stamina (with a few furry friends on hand to cheer us on). Between the fun, we got down to business: a session on our strategy and progress across the firm, how we're putting AI and data to work, and a refresh of our learning and development programme. It's rare to get everyone, across consulting and corporate teams, in one room, and the conversations you have in person are the ones that don't happen over Teams. Our people make our business, and these 24 hours were a chance to reconnect and strengthen our focus on what we're here to do: to have an enduring positive impact on health and healthcare. Thank you to everyone who made it happen. Here's to the next one!
