ABSTRACT Background: Widespread pain is noted in many patients with chronic fatigue syndrome (ME/... more ABSTRACT Background: Widespread pain is noted in many patients with chronic fatigue syndrome (ME/CFS), fibromyalgia and temporomandibular disorders. These conditions usually start as a localized condition and spread to a widespread pain condition with increasing illness duration. Purpose: To aim was to assess the changes in biochemistry associated with pain expression and alterations in renal function. Methods: Forty-seven ME/CFS patients and age/sex-matched controls had a clinical examination, completed questionnaires, standard serum biochemistry, glucose tolerance tests and serum and urine metabolomes in an observational study. Results: Increases in pain distribution were associated with reductions in serum essential amino acids, urea, serum sodium and increases in serum glucose and the 24-hour urine volume; however the biochemistry was different for each pain area. Regression modelling revealed potential acetylation and methylation defects in the pain subjects. Conclusions: These findings confirm and extend our earlier findings. These changes appear consistent with repeated minor inflammatory-mediated alterations in kidney function resulting in essential amino acid deprivation and inhibition of protein synthesis and genetic translation within tissues.

The second objective is to investigate for any correlation between smoking and haem concentration. Clinical parameters were recorded for two healthy and two diseased sites from 22 patients with untreated periodontitis. Gingival crevicular fluid samples were collected from the same sites. Haem assay analysis was undertaken to determine haem concentration at these sites. Gingival crevicular fluid haem concentration was higher at periodontally diseased sites compared to healthy sites (mean 46.6 ± 70.6 nm in healthy vs. 1116.6 ± 2007.0 nm in diseased sites, p < 0.001) and positively correlated with probing pocket depth, attachment level and radiographic bone loss. The initial data from this study indicate that there are clearly identifiable chronic muscle pain... more The initial data from this study indicate that there are clearly identifiable chronic muscle pain conditions in the form of localized pain; myofascial pain or regional pain conditions; and fibromyalgia or generalized pain conditions. A clear difference exists between the prevalence of these conditions in male and female patients, with a higher percentage of female patients suffering generalized pain problems and temporomandibular problems. Generalized or localized pain appears to be an individual variant of a similar problem and pain patients may have a genetically determined vulnerability associated with bacterial toxins, particularly within the genitourinary tract. It appears that in fibromyalgia there is an underlying genetic factor that causes abnormalities in the muscle metabolic cycle, and preliminary data suggest that lipid anomalies predispose to fibromyalgia and possibly chronic fatigue syndrome. The orofacial muscle pain group reported pain in an axial skeletal distribution; neurocognitive, gastrogenitourinary, and musculoskeletal symptoms; infectious events at or preceding onset; similar symptoms in sexual partners; and low prevalence of trauma. Sudden onset was reported by 30.2% of pain patients. Strong associations were found between chronic orofacial muscle pain and (1) onset-related infectious-like events (67.4%); (2) a higher prevalence of history of respiratory and gastrogenitourinary infectious events; and (3) high prevalences of similar pain symptoms in long-term sexual partners. Twenty-nine patients defined by the research diagnostic criteria/TMD as having Type 1a muscle pain (TMD1A group), and 34 age- and sex-matched control subjects, were assessed for variation in urinary organic and amino acid excretion by gas chromatography-mass spectrometry. Twenty-nine patients defined by the research diagnostic criteria/TMD as having Type 1a muscle pain (TMD1A), and 34 age- and sex-matched control subjects were assessed for the carriage of staphylococcal species, staphylococcal toxin production, expression of symptoms, and changes in urinary excretion of amino and organic acids. TMD1A patients had an increased incidence of carriage of toxin-producing coagulase-negative staphylococcus (MDT-CoNS, P < .004), which produced increased levels of delta-like membrane-damaging toxins. Preliminary Determination of a Molecular Basis to Chronic Fatigue Syndrome

Chronic fatigue syndrome (CFS/ME) is a debilitating fatigue illness that has an unknown etiology.... more Chronic fatigue syndrome (CFS/ME) is a debilitating fatigue illness that has an unknown etiology. We studied 20 chronic fatigue syndrome (CFS) patients, who complied with the Oxford and American CDC definitions, and 45 non-CFS subjects. Participants completed questionnaires, were clinically examined, and had first morning urine specimens collected, which were screened by gas chromatography-mass spectrometry for changes in metabolite excretion. Multivariate analysis of the urinary metabolite profiles differed significantly in the CFS patients compared to the non-CFS patients (P &amp;amp;lt; 0.004). The CFS patients had increases in aminohydroxy-N-methylpyrrolidine (P &amp;amp;lt; 0.00003, referred to as chronic fatigue symptom urinary marker 1, or CFSUM1), tyrosine (P &amp;amp;lt; 0.02), beta-alanine (P &amp;amp;lt; 0.02), aconitic acid (P &amp;amp;lt; 0.05), and succinic acid (P &amp;amp;lt; 0.05) and reductions in an unidentified urinary metabolite, CFSUM2 (P &amp;amp;lt; 0.0007), alanine (P &amp;amp;lt; 0.005), and glutamic acid (P &amp;amp;lt; 0.02). CFSUM1, beta-alanine, and CFSUM2 were found by discriminant function analysis to be the first, second, and third most important metabolites, respectively for discriminating between CFS and non-CFS subjects. The abundances of CFSUM1 and beta-alanine were positively correlated with symptom incidence (P &amp;amp;lt; 0.01 and P &amp;amp;lt; 0.001, respectively), symptom severity, core CFS symptoms, and SCL-90-R somatization (P &amp;amp;lt; 0.00001), suggesting a molecular basis for CFS.

These changes in RA can be explained by the well-established inappropriate increase in free radical generation. The similar changes in CFS suggest a similar explanation and a possible role for free radicals in the aetiology of this condition.

We propose that, in addition to its function of gas exchange, the erythron provides a mechanism f... more We propose that, in addition to its function of gas exchange, the erythron provides a mechanism for the inactivation of reactive oxygen and oxide radicals in vivo. In carrying out this function, individual erythrocytes undergo changes in biochemical and structural properties, which are reflected by shape and functional alterations. The changes indicate damage to the labile components of the red cell and demonstrate the expendable nature of the individual red cell. We propose that a superoxide anion channel allows the transport of superoxide and other free radicals into the red cell, where they are deactivated by the erythrocyte antioxidant system which effectively prevents extensive oxidative damage to tissues.
