We engage and coordinate with network providers, specialists, caregivers, and community partners to expand access to the integrated 24/7 medical, behavioral, and social care and support that children deserve. For families of children with special health care needs, navigating referrals, prescriptions, and ongoing care needs can be challenging. Having a trusted care partner can make a meaningful difference. For years, Kelli’s family faced behavioral health crises, hospitalizations, and uncertainty. Today, her family is experiencing something different: stability, connection, and hope. For years, Kelli’s family lived in constant crisis.
