The industry's most widely used framework for evaluating AI-generated clinical notes was developed in 2012, years before large language models entered healthcare. Suki's researchers put that framework to the test. The findings suggest it may not reliably distinguish between today's AI models: reviewers often disagreed on what constituted a hallucination, and the same notes received meaningfully different scores depending on who evaluated them. As healthcare AI continues to advance, the way we evaluate note quality must evolve alongside it. Transparent, rigorous evaluation shouldn't be optional. It should be table stakes.
