At #SNIS2026, new data is being presented on minimally invasive cSDH management. Drs. William Ares (Endeavor Health) and R. Wong (University of Chicago Medicine) just published a single-institution series of 50 patients (61 chronic subdural hematomas) treated with middle meningeal artery embolization, twist-drill craniostomy, and subdural drain placement, all in a single conscious sedation session. No general anesthesia was required. After treatment, they found -Hematoma volume dropped 42% immediately post-op, 62% at 1 month, and 86% at 3 months -88% of patients were Markwalder grade 0 at one month -Only 1 hematoma (1.6%) needed repeat intervention within 90 days -Zero patients required acute operative rescue -Median length of stay: 3 days The volumetric hematoma measurement across all four time points (pre-op, post-op, 1 month, 3 months) was done using Viz SDH+™, our FDA-cleared automated subdural hematoma quantification platform. The use of Viz SDH+ made this kind of longitudinal comparison practical to do consistently across a 3-year cohort, standardizing volume, diameter, and midline shift measurements without relying on manual estimation at every touchpoint. Dr. Ares' minimally invasive techniques can show benefits ny potentially avoiding the need for general anesthesia, improving procedural efficiency, and ultimately cost savings to the institution. Viz.ai’s Dr Chris Mansi Wants to Win Back the Minutes That Matter Early in his career as a neurosurgeon, Chris Mansi operated on a young woman with an acute subdural hemorrhage, and the surgery could not have gone better. “The case went extremely well,” he recalled. Twelve hours later, she died. “You look back at the reason why, and it wasn’t what we did in the operating room,” he said. “It was the four to five hours it took to get her there.” The loss stayed with him, and it pointed at a problem no scalpel could fix. Mansi had trained in the UK and come to Stanford just as deep learning was taking off. He could see that AI was able to read a brain scan, but the bigger opportunity lay upstream. “If the workflow prior to the patient getting to the operating room wasn’t optimized, the patient outcomes were much poorer than they needed to be,” he said. He and his team mapped the route from scan to treatment and found it ran “between 13 and 56 steps,” any of which could stall. A consistent, automated read of a scan, he realized, could alert the right specialist and start the treatment process before hours slipped away. He pitched the idea in a business school class to Google’s Eric Schmidt, who funded it, and co-founded Viz.ai in 2016. “My career went from neurosurgery to startup entrepreneur,” Mansi said. “I took the leap of faith and went for it.” When the FDA told him he could not lean on an existing device and would have to break new ground, he braced for years of delay. Instead, “from that meeting to clearance was four months.” In 2018, Viz.ai earned the first clearance of its kind for an AI care-triage platform, then became the first AI company to win Medicare reimbursement. “We took the right path versus the easy path,” he said. The product is what Mansi calls “an AI safety net,” software that runs around the clock, flags a stroke or aneurysm or rare cardiac condition in seconds, and alerts the specialist who can act. It spread fast because it helped patients and hospital margins at once. Viz.ai now runs in close to 2,000 hospitals, holds more than 50 FDA-cleared algorithms across brain, heart, and lung conditions, and has raised nearly $300 million on its way to becoming a leader in healthcare AI. “Our mission is to increase access to life-saving treatments,” Mansi said, and he wants Viz.ai to one day enhance care for “every single patient and every single test” in the hospital. For the surgeon who once lost a patient to the clock, the work is still about minutes. Now he is trying to win them back by the thousands.
