Daily member relationships. Proven outcomes. In one platform.

About us

Wellth is the daily member relationship platform, proven across millions of members. Healthcare assumes people act in their own best interest. But everyday stress gets in the way, and for members managing chronic conditions, good habits are hard to build and harder to keep. Wellth changes that. We partner with Medicare Advantage, D-SNP, Medicaid, and commercial plans to build a daily relationship with every member, grounded in behavioral science and validated in a Penn Medicine randomized controlled trial. We handle the full journey, from onboarding and activation to bringing members back day after day. Instead of one more vendor pinging members out of context, Wellth becomes the one positive, consistent touchpoint that helps people build the daily behaviors that drive clinical, cost, and quality outcomes: taking blood pressure readings, testing blood sugar, using connected devices, eating healthy meals, managing their care plans, and more. Proven across diverse populations: • 90% average care plan adherence • 45% reduction in post-ACS hospital readmissions (Penn Medicine RCT) • 43% average reduction in high-cost utilization • 51% lower inpatient admissions • 2-4x ROI on cost-of-care reduction • 4+ Star Ratings across Part C and D measures 70 million daily check-ins and counting. Doubling down on a daily member relationship is going to pay dividends for most MA plans. Here's why: 1️⃣ CAHPS and HOS now carry weight most plans aren't ready for. You can't survey your way to a better score. Members answer those questions based on how they feel about their plan, and that feeling gets built (or destroyed) every day of the year. 2️⃣ A member who hears from their plan once a quarter, usually being asked to do something, doesn't report feeling supported. A member with a positive daily touchpoint does. 💡 Wellth members carry NPS scores of 75 to 88+ across programs, and that sentiment is exactly what CAHPS and HOS measure. Watch the highlights from our recent Star Ratings webinar.

  • CAHPS isn't a survey problem. It's a relationship problem. That's the takeaway from Julie Smith, MPH, BSN, RN, Chief Clinical Officer at Sanford Health Plan, on our recent Star Ratings webinar, where she shared how Sanford Health Plan is approaching the measures plans can't control administratively anymore. Here's what the daily relationship looks like in practice: Wellth members average 1,100+ touchpoints a year. Daily check-ins that build trust, then surface the right action at the right moment: an A1C check, a blood pressure reading, a doctor's appointment, or one of many other care gaps closed. The result at Sanford: improvement across Part C and D quality measures, including CAHPS and HOS, and a member NPS of +73. When members feel that way about a daily touchpoint from their plan, the survey takes care of itself.

Managing a self-funded health plan doesn’t have to mean drowning in claims, compliance, and endless admin. That’s where TPAs (third-party administrators) step in—streamlining operations, cutting costs, and ensuring compliance, while helping plans deliver stronger outcomes. But how do TPAs compare to insurers or ASOs? And why are they more important than ever? Success with HOS isn’t really about the survey itself, it’s about everything that happens around it. The plans that excel don’t wait for survey season. They’re focused year-round on the everyday touch points that shape how members feel, engage, and manage their health.
