This is the most structurally interesting PBM announcement of 2026 — and I don't think it's being read that way yet. Every other move in the breakaway group this year has been about data infrastructure or scale. Judi Health building unified medical-pharmacy claims. Abarca and LucyRx combining for commercial reach. Platform plays — build the foundation, layer clinical intelligence on top later. SmithRx and Form Health skipped that sequence. Transparent pharmacy economics plus physician-led prescribing oversight, deployed commercially, in the one category where which drug for which patient is most urgent and most expensive right now. That's not a cost program with a clinical wrapper. That's clinical logic leading and pharmacy economics following — the inversion the legacy PBM model has never made. The standard PBM response to GLP-1 pressure has been PA criteria and utilization management. This puts a physician in the decision before the pharmacy claim exists — and removes the rebate incentive to favor the higher-cost option. Curious whether Form Health is seeing different adherence and discontinuation patterns managed this way versus standard PBM coverage. That outcome data is the most valuable asset this partnership produces.
