Private equity isn’t transforming orthopaedics and spine because it’s the ideal model. It’s expanding because many physicians feel they have few better alternatives. When independent practice becomes increasingly difficult to sustain, capital inevitably fills the void. Rather than debating whether private equity is good or bad, we should ask a more fundamental question: Why have we created an environment where so many successful physician practices believe selling is their best path forward? If we want physician-led care delivery models to remain viable, we need payment models and regulatory policies that reward value, innovation, and accountability, not just size and administrative scale. The future of musculoskeletal care shouldn’t be determined by who has the deepest pockets. It should be determined by who consistently delivers the best outcomes, patient experience, and value. Immediate Weight-Bearing After Tibial Plateau Fractures Internal Fixation Results in Better Clinical Outcomes with Similar Radiological Outcomes: A Randomized Clinical Trial Our findings suggest that immediate weight-bearing after stable fixation can improve clinical recovery without compromising radiological outcomes, supporting a more evidence-based approach to postoperative rehabilitation. This work reflects my research interest in bridging orthopedic surgery and rehabilitation, ensuring that postoperative protocols are guided by evidence rather than tradition. The most meaningful lessons in medicine aren't always found in a textbook. As keynote speaker for the McGovern Medical School Class of 2026, Rex Marco, MD, professor of orthopedic surgery and director of the Orthopaedic Spine Fellowship at UTHealth Houston, shared a message that extended far beyond clinical practice. Drawing from his own experiences as a surgeon, patient and educator, Dr. Marco encouraged graduates to practice gratitude, choose forgiveness and lead with love throughout their careers. Continuous learning is one of the foundations of excellence in Orthopaedic Surgery. I recently had the opportunity to attend the Summer School on Bone Metabolism, held in Kalamata, Greece, bringing together experts and healthcare professionals with a shared interest in bone health and metabolic bone diseases. The program provided valuable insights into the latest advances in bone biology, osteoporosis, fracture healing, and the multidisciplinary management of metabolic bone disorders. It was an excellent opportunity to deepen my knowledge, exchange ideas with colleagues, and stay updated with the latest scientific developments. As an Orthopaedic Surgery resident, I strongly believe that investing in lifelong learning is essential for providing the highest standard of patient care. GLP-1 receptor agonists like #semaglutide and #tirzepatide are changing more than diabetes and weight management. Their effects are also being felt in orthopaedic care. Justin Haghverdian, MD, a UCI Health orthopaedic surgeon specializing in foot and ankle surgery, is seeing patients achieve significant reductions in weight and hemoglobin A1C while taking these medications. For patients who require surgery, those improvements may help optimize their health before a procedure. And in some cases, weight loss may also reduce pain enough that surgery is no longer necessary. For Haghverdian, these developments illustrate the value of practicing at an academic medical center, where physicians collaborate directly with experts across specialties and incorporate emerging approaches into their own fields. “Through research, seeing these outcomes in real time and talking with our experts, we’re able to translate what they are doing into our own field and advance patient care.”
