Health Tips by Myra: Facts About your Medical Records 1-Doctors (medical providers) DO NOT read ALL the medical records you gave them. 2-Your specialists (and often your primary care) typically don’t communicate with each other. 3-Medical records can be destroyed if inactive, after 7 years. We moved to NJ from Maryland 1 year ago, and we are still in the process of organizing our medical records and getting new providers. My first step was contacting our primary care physician to get our records (going back ~ 20 years). Between my husband and myself, they were each between 70 and 100 pages, with many duplicate pages. (Every time we were seen, they cut and pasted, with all of our medications, problem lists, and immunizations.) In addition, there was NOTHING from any of the specialists we had seen over the years, and I know for a fact that notes were sent. When you request records, doctors will typically not send any records from other physicians, tests they performed, or hospitalizations they were not involved in. This is why you must have all of your medical records in order and learn to advocate for yourself. As I have said repeatedly, we all need a goal for every visit. My husband recently went to a new urologist. Before we went, I typed up a one-page summary for the physician. It included his entire medical history (condensed) with one paragraph for his urological history. We also brought his notes from the last visit with the Maryland doctor. The new doctor was thrilled, he said we made his life much easier, and it was a very successful visit, and goals were met. You don’t need my medical background to do this, but if you need to, ask someone. Ask an advocate, but know your medical history. Unfortunately, Dr. Marcus Welby no longer exists, health care is much more complicated, and the one who cares most is you! Please, get all of your records in order. If you are overwhelmed, get help.
