New post-discharge program connects eligible members with coordinated care, provider support, and local resources where they are, when they need them. Priority Telemedicine Group, PA powered by OnDemand Visit announced today that Arizona Complete Health and its network of programs have partnered with Priority Telemedicine Group, PA to launch a new post-discharge care management program designed to support eligible members for up to 30 days after leaving the hospital or skilled nursing facility. Through this partnership, OnDemand Visit will help members navigate the critical period after discharge by coordinating support through its suite of MD-led providers, care team specialists, and local paramedics. The program is designed to help members better understand their discharge instructions, follow-up needs, medications, available resources, and next steps as they recover outside of the hospital. Through its multi-channel approach, OnDemand Visit aims to visit patients within three (3) days of being discharged from the hospital or skilled nursing facility. “We understand that the 30 days following discharge are vital as members begin recovery outside of the facility setting whether it be the hospital or skilled nursing facility,” said Dr. Chanchal Yadav, Chief Medical Officer of Arizona Complete Health. “We are excited for this partnership with OnDemand Visit because it brings additional support and resources to our members during a critical recovery period.
