Just in time for #ClimateWeekNYC our JAMA paper on reducing healthcare plastics use and pollution is hot off the press. Plastics are ubiquitous and overused in healthcare, and nearly a quarter of the 14,000 tons of waste generated daily by US hospitals is plastics, much of it single-use plastics such as syringes, wrappings, IV bags, catheters, test kits, masks, and gloves. These end up in landfills or infiltrate the environment, contributing to harmful chemicals and microplastics in humans which lead to adverse health outcomes. Did you know that 98% of chemicals in plastics are derived from fossil fuels? And plastics production accounts for nearly 4% of global greenhouse gas emissions and is bound to increase unless we take action. Healthcare should lead efforts to curb plastics pollution and protect human health. To achieve this, Hilary Ong Cassandra Thiel and I propose a comprehensive sociotechnical systems approach consisting of interrelated technical and non-technical intervention components that can inform mitigation strategies across the entire plastics lifecycle: production, design, use, waste generation, and waste management. We recommend strategies directly focused on reducing use and waste generation at the point of clinical care and indirect strategies to address other aspects of the plastics lifecycle (i.e., reuse, recycle, research, and redesign to reduce reliance on plastics based on fossil fuels). We have a breakthrough from Africa: Nature’s solution to plastic pollution 🌱 Plastic pollution is one of the most pressing environmental challenges of our time. Traditional recycling methods struggle to address the sheer scale of the problem, particularly in Africa, where importation of plastic products is high, and recycling infrastructure is limited. But nature might just have a solution. The findings from a groundbreaking study by the International Centre of Insect Physiology and Ecology (ICIPE): Kenyan lesser mealworm larvae have been discovered to consume polystyrene (commonly known as Styrofoam)- a notoriously difficult plastic to break down. Here’s what makes this discovery so exciting: 1. The Role of Gut Bacteria: These mealworms host bacteria in their guts that produce enzymes capable of breaking down polystyrene. Bacteria like Kluyvera and Klebsiella were particularly abundant in polystyrene-fed larvae, highlighting their potential in managing plastic waste. 2. A Balanced Diet for Efficiency: Mealworms fed a combination of polystyrene and nutrient-rich bran broke down plastic more effectively than those on a polystyrene-only diet. This finding underscores the importance of maintaining insect health for optimal waste management. 3. An African Innovation: Unlike previous studies on plastic-eating insects, this research focuses on a species native to Africa, offering tailored insights for tackling the continent’s unique plastic pollution challenges. This discovery is a testament to the power of nature and science working hand in hand to address global challenges. With plastic straws making a comeback in the U.S., it's a good time to take stock of India's case and delve deeper. We generate approx 9.3 million tonnes of plastic waste annually, nearly one-fifth of the global total. This issue is particularly relevant given the rapid growth of e-commerce. In 2023 alone, e-commerce companies used nearly 1.2 million tonnes of plastic, and the industry is expanding at a CAGR of over 18%. The food delivery sector is another major contributor, generating approximately 350,000 tonnes of single-use plastic waste annually. Rajeev's recent post highlighted an interesting perspective—policies like reintroducing plastic straws in the U.S. may not have an immediate domestic impact because much of their plastic waste is exported to countries like India. This means India is not only struggling with rising domestic plastic consumption but also dealing with increasing plastic imports. So, what could be the solution atleast on domestic consumption side? Clearly, paper-based substitutes aren’t effective. Scientists are now exploring algae-based derivatives as biodegradable alternatives to plastic. Algae offers several advantages—it is abundant, grows rapidly, and can be cultivated without harming the ecosystem. Unlike paper packaging, which requires cutting down trees and disrupts carbon absorption, algae provides a sustainable and eco-friendly alternative without depleting natural resources. Two amazing startups in the global stage which are innovating in this space: Notpla (UK): Notpla has developed seaweed-based packaging that naturally biodegrades within weeks, unlike plastic, which lingers for centuries. Their products include edible water pods, biodegradable takeaway containers, and sustainable food packaging coatings. Loliware (USA): Loliware is another pioneer in seaweed-based, fully compostable alternatives to plastic. Their product line includes edible straws and cutlery designed to replace single-use plastics in the food and beverage industry. While global innovation in algae-based alternatives is advancing, India needs to accelerate progress in this area. This includes adopting global best practices, investing in domestic research in microbiology and material science, and strengthening regulatory support to encourage startups and industries to transition to sustainable packaging solutions. By fostering innovation and policy-driven change, India can significantly reduce its plastic waste burden and move toward a more sustainable future. 🌍 As the world moves (hopefully) toward finalising the UN Global Plastics Treaty, a group of international experts — including myself — are calling for health to be placed at the centre of plastics policy. Our new Health Policy in the Lancet reviews growing evidence that plastics pose risks to human health across their entire life cycle — from emissions at production sites and chemical exposures in consumer products, to microplastics detected in human tissues and open burning of plastic waste contributing to air pollution. 📊 Key figures: 1️⃣ Global plastic production has increased from 2 million tonnes in 1950 to 475 million tonnes in 2022, with projections reaching 1.2 billion tonnes by 2060. 2️⃣ Only 9% of plastic is currently recycled, and an estimated 8 billion tonnes of plastic waste remain in the environment. 3️⃣ Production releases over 2 gigatonnes of CO₂ equivalent per year, contributing significantly to climate change and its health impacts. 4️⃣ There are documented health effect across the entire plastic life cycle from production, associated chemicals, use, and waste. 5️⃣ In many low- and middle-income countries, 57% of unmanaged plastic waste is burned in the open — a major source of harmful air pollution. 6️⃣ The global health-related costs of plastic pollution are estimated to exceed US $1.5 trillion annually. 📢 To support evidence-based policymaking, we are launching the Lancet Countdown on Health and Plastics — a global monitoring initiative that will track plastic-related health impacts and guide actions as the treaty comes into force. The negotiations on a global plastics treaty (INC 5.2) resume in Geneva in early August. Our International Science Council Expert Group on Plastic Pollution, comprised of 16 experts from around the world, co-authored a short set of science-based priorities for the upcoming negotiations. Our paper in Nature Sustainability outlines the following four key elements for an effective #treaty and related provisions in the Chair’s negotiating text for further scientific input: 1️⃣ A global target that limits primary plastic production to sustainable levels that is grounded in science and regularly reviewed. 2️⃣ Science-based criteria and phase-out timelines for harmful chemicals and problematic plastic products. 3️⃣ A framework for robust monitoring and effectiveness evaluation, built on standardized protocols and open-access data. 4️⃣ A strong science–policy–society interface that is independent, interdisciplinary, inclusive, and policy-relevant. We call on Member States to include binding provisions that address currently unsustainable levels of plastic production and related harmful impacts, such as from chemical substances, and to integrate diverse knowledge systems—including Indigenous and local expertise—into the treaty's design, development, and implementation. This is a crucial moment for taking a stand against ever-increasing levels of global plastic pollution and its harms to people, wildlife, and our environment. Crucial multilateral negotiations are underway in Busan, South Korea, to craft a new global treaty governing the production, use and disposal or reuse of plastics. Already a huge pollution problem that threatens the health of humans and of ecosystems, there are staggering and reliable projections that show plastics production is likely to continue its rapid increase. While consumer demand due to the convenience of these ubiquitous products is at the heart of this expansion, we are not paying the full price of their production and management. Recycling as currently practiced and promoted is mostly greenwashing. Taxpayers end up footing the bill through disposal and cleanup costs, as well as from increasingly documented adverse health impacts. It is also becoming clear that oil and gas companies and petro-states are shifting their emphasis to plastics production because they see them (and other non-energy petrochemical products) as fallback revenue streams in the face of pressures - driven by a response to the climate change crisis - to phase out reliance on fossil fuels for energy. While this phase out is slower than needed to halt the rise of global temperatures and extreme weather, shifts in policy, markets and technologies will eventually make most oil and gas production uneconomic. So it’s crucial that a strong new plastics treaty be crafted and agreed to set clear and binding rules for the industry. This should not only ensure that the carbon footprint of plastics is measured and abated, but that the industry embraces alternatives to plastics (including bans on most single-use products), restrictions on those chemicals that are most harmful to health or difficult to reuse or recycle, industry responsibility for tracking and proper reuse, recycling or disposal of their products and other sustainability measures.
