According to the British Dental Association, 86% of UK dentists have treated complications from dental work done abroad. The problem isn't any one country. There are excellent dentists everywhere. The problem is conveyor belt dentistry. I've seen patients with teeth prepared to pegs, gum disease developing underneath because the preparation was rushed and careless. Damage that cannot be undone. They are now spending far more than they saved trying to correct it. I've seen patients whose results are simply ugly. Big blocks of white that don't belong in that face. In some of these cases there is something I can do. Recontour the ceramics, work with what's there. It's not always possible. The difference between these outcomes and a great result isn't which country you're in. It's whether the dentist treating you is thinking about your face, your tissue, your long-term oral health, or clearing the chair for the next patient. Quality dentistry takes time. You can predict which treatment plans will convert before the patient leaves the chair. Three things tell you almost everything: 1. Can the patient explain the problem back to you in their own words? 2. Have they actually seen it — a photo, a scan, an X-ray on the screen? 3. Do they understand the cost of doing nothing? Notice what's not on that list: how well you explained the dentistry. We assume acceptance comes down to the quality of our explanation. It doesn't. A patient can nod along to a brilliant explanation, understand none of it, and walk straight out the door. Acceptance depends on what the patient understands — not on how well you explain. Next time, before you talk cost, check for those three points. If any are missing, you already know how the conversation ends. PFM crowns still have a place in restorative dentistry. But knowing when to reach for one versus a full-ceramic alternative is worth revisiting. Where PFM tends to hold up well: - Long-span bridges where flexural strength is the priority - Cases with limited ceramic space and high occlusal load - Patients where cost is a significant factor and esthetics are secondary Where full-ceramic options tend to serve better: - Anterior esthetic cases where translucency matters - Patients with metal sensitivities - Cases where the gingival margin appearance over time is a clinical concern The real mistake is not choosing one over the other — it is choosing based on habit rather than the specific clinical picture in front of you. Every patient case brings its own set of variables. Material selection is most reliable when it follows the clinical indicators, not the path of least resistance.
