One of the biggest challenges with Alzheimer’s is that by the time symptoms become noticeable, the disease has often been progressing for quite some time. A new study suggests a blood test may one day identify people at high risk of developing Alzheimer’s before any symptoms develop. While the test isn’t ready for routine use, the direction of the research is promising. Earlier detection could allow families to prepare sooner, physicians to monitor patients more closely, and, as new therapies continue to emerge, create opportunities to intervene before significant cognitive decline occurs. Some of the most meaningful advances in medicine come from identifying disease earlier not just treating it after symptoms begin. As the global scientific and healthcare community convene at #AAIC26 this week, the conversation around #Alzheimers feels fundamentally different from even a few years ago. We are seeing a vital shift in focus: from managing late-stage symptoms to recognising the profound power of proactive, early diagnosis. We now know that Alzheimer’s pathology begins years and sometimes even decades before clinical memory loss becomes apparent. Unlocking the ability to reliably detect these underlying biological changes early is perhaps the most crucial puzzle piece in our current landscape. Advanced diagnostics pave the way for intervention when it matters most. Clarity can replace uncertainty, giving patients and their families back a much-needed sense of control. New research, partially funded by the Alzheimer’s Association and presented at #AAIC26, suggests a future where primary care physicians could use a highly accurate blood test to help diagnose—or rule out—Alzheimer's in people experiencing memory and thinking problems. In a head-to-head comparison, researchers found blood test results enabled primary care physicians to diagnose Alzheimer’s with nearly the same accuracy (90 percent) as specialists. Knowing the blood test results led doctors to change diagnoses or care management decisions for more than half of primary care patients and nearly half of specialty care patients. Here’s why that gives us hope: People experiencing memory and thinking problems often face long waits and multiple referrals before receiving a diagnosis. As part of a doctor's evaluation for memory and thinking problems, accurate blood tests could help bring answers sooner—opening the door to earlier and better treatment, care and planning.

Alzheimer’s disease and cognitive impairment are growing challenges for patients, families, and health systems across the country. But earlier detection can make a meaningful difference. University of Utah Health is joining the Davos Alzheimer's Collaborative U.S. Early Detection Expansion Program to help make cognitive screening a more routine part of primary care at five clinics. Led by Timothy Farrell, MD, the effort will focus on identifying cognitive changes earlier, supporting patients and families sooner, and building sustainable care pathways rooted in age-friendly care. As Dr. Farrell explains, age-friendly care centers on the “4Ms”: what matters most to the patient, medications, mentation, and mobility. That approach helps care teams understand not just a diagnosis, but how cognitive changes affect daily life, independence, and family support. For a many years we have treated Alzheimer’s strictly as an amyloid-clearance problem. New real-world data from AAIC 2026 demands we look upstream. A large-scale study (4,940 propensity-matched dementia patients per cohort via TriNetX) revealed a profound shift: GLP-1RA users saw an 18.2% all-cause mortality rate compared to 21.1% in non-users (HR 0.67, p<0.001). While healthier-patient confounding always exists, this data strongly suggests that the true lever for slowing neurodegenerative progression might be vascular and metabolic, not just plaque.
